益肾排毒方治疗老年糖尿病肾病患者疗效及对24h尿蛋白与β2微球蛋白的影响

摘　　要：目的 探讨益肾排毒方治疗老年糖尿病肾病患者疗效及对24 h尿蛋白、β2微球蛋白的影响。方法 将武汉市中医医院2019年1月—2021年4月收治的老年糖尿病肾病患者101例,随机对照表法随机分为对照组53例和观察组48例。对照组采用降糖、降压、控制蛋白、纠正电解质代谢紊乱、肾性贫血及肾性骨病等对症治疗;在此基础上,观察组采用益肾排毒方治疗,两组均治疗12周。比较两组的临床疗效及不良反应状况,测定白细胞介素-6(interleukin-6,IL-6)、C反应蛋白(C-reactive protein, CRP)、血肌酐(serum creatinine, Scr)、尿素氮(urea nitrogen, BUN)及24 h尿蛋白(24 h urine protein quantification, 24 h Upro)、β2-微球蛋白(β2-microglobulin, β2-MG)。结果 观察组治疗后血清IL-6与CRP低于对照组,差异有统计学意义(P<0.05)。观察组治疗后SCR、BUN及24 h Upro、β2-MG水平低于对照组(P<0.05)。对照组有效率79.25%(42/53)与观察组93.75%(45/48)比较,差异有统计学意义(P<0.05)。对照组总不良率18.87%(10/53)与观察组27.08%(13/48)比较,差异无统计学意义(P>0.05)。结论 益肾排毒方对老年糖尿病肾病患者的疗效显著,可降低患者炎症因子水平,降低24 h Upro、β2-MG,改善肾功能和血管内皮功能,值得推广。Objective To investigate the effect of Yishen Paidu Formula(益肾排毒方)on elderly patients with diabetic nephropathy and the influence on 24 h urinary protein andβ2-microglobulin.Methods A total of 101 elderly patients with diabetic nephropathy in the hospital from January 2019 to April 2021 were randomly divided into the control group(53 cases)and the observation group(48 cases).The control group was treated with symptomatic treatment such as hypoglycemic treatment,blood pressure control,protein control,correction of electrolyte metabolism disorder,renal anemia and renal bone disease.On this basis,the observation group was treated with Yishen Paidu Formula.Both groups were treated for 12 weeks.The clinical efficacy and adverse reactions of the two groups were compared,and the levels of interleukin-6(IL-6),C-reactive protein(CRP),serum creatinine(Scr),urea nitrogen(BUN)and 24 h urine protein quantification(24 h Upro)andβ2-microglobulin(β2-MG).Results After treatment,the serum levels of IL-6 and CRP in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of SCR,BUN,24 h Upro andβ2-MG in the observation group were lower than those in the control group(P<0.05).The effective rate of the control group was 79.25%(42/53)and that of the observation group was 93.75%(45/48),and the difference was statistically significant(P<0.05).There was no significant difference in total defect rate between the control group(18.87%,10/53)and the observation group(27.08%,13/48)(P>0.05).Conclusion Yishen Paidu Formula has a significant effect on elderly patients with diabetic nephropathy.It can reduce 24 h Upro andβ2-MG and improve the renal function and vascular endothelial function,worthy of promotion.
