基于倾向性评分匹配法的祛痹化纤方治疗特发性肺间质纤维化疗效评价

摘　　要：目的 通过真实世界研究方法评价祛痹化纤方治疗特发性肺间质纤维化(IPF)的临床疗效。方法 选取2013年1月1日—2020年12月31日在北京中医药大学东方医院呼吸科就诊的气阴两虚、痰瘀互结证的IPF患者,根据是否使用祛痹化纤方分为试验组230例和对照组87例。通过倾向性评分匹配(PSM)平衡基线特征后,对两组患者进行疗效评价。结果 经PSM分析后,共82对患者匹配成功。匹配后,两组在基线特征方面差异均无统计学意义。疗效评价:(1)治疗后,试验组胸部CT积分、一氧化碳弥散量(DLCO)、6 min步行距离(6MWD)较治疗前均有明显变化(P<0.05),试验组明显优于对照组(P<0.001);(2)治疗后,试验组中医证候评分较治疗前无明显变化(P>0.05),试验组明显低于对照组(P<0.001)。结论 祛痹化纤方可减轻IPF患者的中医临床症状,延缓肺功能恶化程度,提高活动耐力,值得临床推广和应用。Objective To evaluate the clinical efficacy of Qubi Huaxian Formula(祛痹化纤方)treating idiopathic pulmonary fibrosis(IPF)via real-world methods.Methods From January 1,2013 to December 31,2020,patients admitted to the respiratory department of Dongfang Hospital of Beijing University of Chinese Medicine were included in this research.All of them were diagnosed as IPF and conformed to Qi and Yin deficiency type or phlegm-blood stasis blocking collateral type.According to whether administered with Qubi Huaxian Formula,patients were divided into experimental group and control group(230 cases vs 87 cases).After balancing the baseline characteristic by Propensity Score Matching(PSM),we assessed the clinical efficacy of the two therapeutic strategies.Results After PSM analysis,82 pairs of patients were matched successfully.After matching,there were no significant differences in baseline level between the two groups.Assessment of clinical efficacy:After treatment,chest CT score,carbon monoxide diffusing capacity(DLCO)and 6-minute walking distance(6MWD)of the experimental group were significantly different from those before treatment(P<0.05)and those of the experimental group were significantly better than those of the control group(P<0.001).After treatment,the TCM syndrome score of the experimental group had no significant change(P>0.05)and the score of the experimental group was significantly lower than that of the control group(P<0.001).Conclusion QubiHuaxian Formula can relieve the clinical symptoms of IPF,delay the deterioration of lung function and improve the endurance of activity.It is worthy of popularizing and applying in clinical practice.
