健脾补肾联合淋巴细胞主动免疫治疗不明原因复发性流产的效果

摘　　要：目的探讨健脾补肾联合淋巴细胞主动免疫治疗不明原因复发性流产(URSA)的疗效。方法收集198例URSA患者病例资料,随机分为淋巴细胞主动免疫治疗组(66例)、健脾补肾治疗组(66例)及联合治疗组(66例),分析比较三组研究对象的妊娠结局。结果联合治疗组的封闭抗体(BA)转阳率、保胎成功率、血清雌二醇(E2)、人绒毛膜促性腺激素(β-HCG)水平、治疗后血清白介素10(IL-10)水平均高于免疫治疗组和健脾补肾治疗组(P<0.01),联合治疗组妊娠率高于免疫治疗组(P<0.017),联合治疗组治疗后的中医证候积分、白介素(IL-2)水平低于免疫治疗组和健脾补肾治疗组(P<0.01)。健脾补肾治疗组治疗后中医证候积分低于免疫治疗组(P<0.01)。三组研究对象治疗后中医证候积分、血清IL-2水平低于治疗前(P<0.01),血清IL-10水平高于治疗前(P<0.01)。结论健脾补肾联合淋巴细胞主动免疫治疗URSA可明显改善妊娠结局,具有良好的治疗潜力,而单独健脾补肾与淋巴细胞主动免疫治疗URSA相比,可改善中医证候,其他妊娠结局相当,无明显差异。Objective To explore the effects of spleen invigorating and kidney tonifying combined with lymphocyte immunotherapy on unexplained recurrent spontaneous abortion(URSA).Methods Data of 198 URSA patients were collected.They were randomly divided into the lymphocyte immunotherapy group(66 cases),the spleen invigorating and kidney tonifying group(66 cases),and the combined treatment group(66 cases)for the analysis of the pregnancy outcomes.Results The rate of positive blocking antibody,success fetal protection and the level of E2,β-HCG and IL-10 after the treatment in the combined treatment group were higher than those in the lymphocyte immunotherapy group and the spleen invigorating and kidney tonifying group(P<0.01).The pregnancy rate in the combined treatment group was higher than that in the lymphocyte immunotherapy group(P<0.01).The TCM syndrome score and the level of IL-2 after the treatment in the combined treatment group were lower than those in the lymphocyte immunotherapy group and the spleen invigorating and kidney tonifying group(P<0.01).After the treatment,the TCM syndrome score of spleen invigorating and kidney tonifying group was lower than that of the lymphocyte immunotherapy group(P<0.01).After the treatment,the TCM syndrome score and the level of IL-2 in serum of the three groups were lower than those before the treatment and the level of IL-10 in serum was higher than that before the treatment(P<0.01).Conclusion Spleen invigorating and kidney tonifying combined with lymphocyte immunotherapy on URSA can significantly improve pregnancy outcome and has good therapeutic potential.Compared with lymphocyte immunotherapy URSA,invigorating spleen and kidney tonifying can improve TCM syndromes,and other pregnancy outcomes are similar,with no significant differences.
