sST2、Lp-PLA2、BNP与慢性心力衰竭患者心功能及预后的关系研究

摘　　要：目的研究可溶性ST2(sST2)、脂蛋白相关磷脂酶A2(Lp-PLA2)、脑钠肽(BNP)与慢性心力衰竭患者心功能及预后的关系。方法回顾性分析2019年2月—2020年8月于该院治疗的107例慢性心力衰竭患者的临床资料,设为研究组,另选取同时期进行健康体检患者82例,作为对照组。比较不同人群sST2、Lp-PLA2、BNP水平,分析影响慢性心力衰竭患者预后的危险因素,绘制ROC曲线,计算sST2、Lp-PLA2联合BNP检测预测慢性心力衰竭患者预后的效果。结果研究组sST2、Lp-PLA2、BNP水平均高于对照组(P<0.05)。随着心功能级别越高,sST2、Lp-PLA2、BNP水平逐渐升高(P<0.05);经非条件多因素logistic回归模型分析得,心功能分级、sST2、Lp-PLA2、BNP为影响慢性心力衰竭患者预后的独立危险因素(P<0.05);ROC曲线示:sST2、Lp-PLA2、BNP单独检测以及三者联合检测AUC分别为0.797、0.825、0.848、0.939,以三者联合检测AUC值(95%CI:0.896~0.983)最大。结论sST2、Lp-PLA2、BNP可作为影响心力衰竭预后的因素,对于心力衰竭的诊断和预后判断有一定临床意义,可再深入扩展研究。Objective To analyze the effects of multi-indicator combined detection in the evaluation of cardiac function and prognosis in patients with chronic heart failure.Methods The clinical data of one hundred and seven patients with chronic heart failure treated in The First Affiliated Hospital of Nanjing Medical University from February 2019 to August 2020 were retrospectively analyzed,and they were set as the research group,and 82 patients who underwent physical examinations during the same period were selected as the control group.The levels of soluble ST2(sST2),lipoprotein-associated phospholipase A2(Lp-PLA2),and BrainNatriuretic Peptide(BNP)in different populations were compared,and the risk factors affecting the prognosis of patients with chronic heart failure were analyzed,and the ROC curve was drawn to calculate the effect of sST2,Lp-PLA2 combined with BNP detection in predicting the prognosis of patients with chronic heart failure.Results The levels of sST2,Lp-PLA2 and BNP in the study group were higher than those in the control group,and the difference was statistically significant(P<0.05).With the higher the cardiac function grade,the levels of sST2,Lp-PLA2 and BNP gradually increased,and the difference was statistically significant(P<0.05);Unconditional multivariate logistic regression model analysis showed that cardiac function class,sST2,Lp-PLA2 and BNP were independent risk factors affecting the prognosis of patients with chronic heart failure(P<0.05);the ROC curve showed that the AUC of sST2,Lp-PLA2 and BNP alone and the three combined detection were 0.797,0.825,0.848,and 0.939,respectively,and the AUC value of the three combined detection(95%CI:0.896~0.983)was the largest.Conclusion sST2,Lp-PLA2 and BNP can be used as factors affecting the prognosis of heart failure,and have certain clinical significance for the diagnosis and prognosis of heart failure,which can be further extended analysis.
