膀胱癌组织中miR-29c-3p的表达与临床病理特征的关系及对治疗预后的影响

摘　　要：目的 探讨膀胱癌组织中miR-29c-3p的表达与临床病理特征的关系及对治疗预后的影响。方法 2013年12月—2016年12月该院接受手术治疗的原发性膀胱癌患者90例,同期膀胱囊肿患者术后正常膀胱组织50例进行对照,对比膀胱癌组织、正常膀胱组织中miR-29c-3p表达水平的差异,采用受试者工作特征(ROC)曲线分析组织中miR-29c-3p对膀胱癌的辅助诊断价值。分析膀胱癌组织中miR-29c-3p表达水平与临床病理特征的关系,以及膀胱癌患者术后随访期复发转移的危险因素。采用Kaplan-Meier生存曲线分析膀胱癌组织中不同miR-29c-3p表达量对随访期无病生存情况的影响。结果 膀胱癌组织中miR-29c-3p的表达水平低于正常膀胱组织(P<0.05)。ROC曲线显示,组织miR-29c-3p表达水平辅助诊断膀胱癌的最佳截断值为0.535,AUC为0.755(95%CI 0.675~0.835),对应的灵敏度、特异度分别为64.00%、72.22%。不同性别、年龄、肿瘤数目的膀胱癌患者组织中miR-29c-3p表达水平的差异无统计学意义(P>0.05);不同肿瘤最大直径、病理分级、淋巴结转移、肿瘤分期膀胱癌患者组织中miR-29c-3p表达水平的差异有统计学意义(P<0.05)。COX回归分析发现:组织中miR-29c-3p低表达、肿瘤最大直径≥3cm、病理分级低级别、合并淋巴结转移、肿瘤分期T2~T3是膀胱癌患者术后复发转移的危险因素(P<0.05)。Kaplan-Meier曲线结果显示,高miR-29c-3p组患者的无病生存情况优于低miR-29c-3p组患者(P<0.05)。结论 膀胱癌组织中miR-29c-3p的表达与临床病理特征关系密切,低表达的miR-29c-3p可促使膀胱癌患者术后复发转移。Objective To investigate the relationship between miR-29C-3p expression and clinicopathological features in bladder cancer tissue and its effect on treatment prognosis.Methods Ninety cases of primary bladder cancer patients underwent surgical treatment in our hospital during December 2013 to December 2016 were collected,50 cases of normal bladder tissues postoperatively in patients with bladder cyst during the same period were collected.MiR-29c-3p expression between two bladder tissue were compared,receiver-operating characteristic(ROC) curve analysis was used to analyze the value of miR-29C-3p in auxiliary diagnosis of bladder cancer.Relationship between expression level of miR-29C-3p in bladder cancer tissues and clinicopathological characteristics,risk factors of recurrence and metastasis in patients with bladder cancer during the postoperative follow-up period were analyzed.Kaplan-Meier survival curve was used to analyze the effects of different miR-29C-3p expression levels in bladder cancer tissues on disease-free survival during the follow-up period.Results Expression level of miR-29C-3p in bladder cancer tissue was lower than that in normal bladder tissue(P<0.05).ROC curve showed that optimal cut-off value of miR-29C-3p expression level in auxiliary diagnosis of bladder cancer was 0.535,AUC was 0.755[95%CI 0.675~0.835],corresponding sensitivity and specificity were 64.00% and 72.22% respectively.There was no significant difference in expression level ofmiR-29C-3p in bladder cancer patients with different gender,age and number of tumors(P>0.05).Expression level of miR-29C-3p in bladder cancer patients with different tumor maximum diameter,pathological grade,lymph node metastasis and tumor stage had statistical significance(P<0.05).Cox regression analysis showed that low expression of miR-29C-3p in tissues,maximum tumor diameter≥3 cm,low pathological grade,complicated lymph node metastasis,tumor stage T2-T3 were risk factors for postoperative recurrence and metastasis of bladder cancer patients(P<0.05).Kap
