摘 要:目的探讨孕中期唐氏筛查血清学指标对妊娠期高血压疾病的预测价值。方法回顾性分析116例产检并顺利分娩产妇的临床资料。按照出院诊断结果将其分为观察组(56例)与对照组(60例)。对比两组产妇游离β-人绒毛膜促性腺激素(freeβ-HCG)、甲胎蛋白(AFP)及游离雌三醇(uE_(3))水平,并计算其联合诊断以及单独诊断的敏感度、特异度及约登指数。结果观察组产妇的freeβ-HCG水平高于对照组,且AFP和uE3水平低于对照组,差异有统计学意义(P<0.05)。将freeβ-HCG、AFP和uE_(3)联合检验因子作为检验变量计算约登指数,freeβ-HCG、AFP和uE_(3)单独诊断和三联诊断的敏感度最高的是AFP和uE_(3),特异度则以freeβ-HCG最高,但三联检验则具有比较平衡的特异度和敏感度,同时三联检验的约登指数最高,AFP最低。结论孕中期唐氏筛查freeβ-HCG、AFP和uE_(3)三种血清学指标对妊娠期高血压疾病具有较好的预测价值,值得临床推广与应用。Objective To investigate the value of Down's screening serological index in predicting Hypertensive diseases of pregnancy.Methods The clinical data of 116 parturients were analyzed retrospectively.According to the discharge diagnosis,the patients were divided into observation group(56 cases)and control group(60 cases).The levels of free human chorionic gonadotropin(freeβ-HCG),alpha-fetoprotein(AFP)and free estriol(uE_(3))were compared between the two groups of pregnant women,and the sensitivity,specificity and Yoden index of combined diagnosis and single diagnosis were calculated.Results The level of freeβ-HCG in the observation group was higher than that in the control group,and the level of AFP and uE_(3) was lower than that in the control group,the difference was statistically significant(P<0.05).The combined test factors of freeβ-HCG,AFP and uE_(3) were used as test variables to calculate the Yoden index.The sensitivity of freeβ-HCG,AFP and uE3 in single diagnosis and triple diagnosis was the highest,while the specificity of freeβ-HCG was the highest,but the triple test had a more balanced specificity and sensitivity.The Yoden index was the highest and AFP was the lowest.Conclusion Down's screening in the second trimester of pregnancy has a good value in predicting Hypertensive diseases of pregnancy,AFP and uE_(3),and is worthy of clinical promotion and application.
