住院病案首页数据对公立医院医疗服务能力的评价探讨

摘　　要：目的 探索通过基于住院病案首页数据的医疗服务能力指标分析,为公立医院重点科室医疗服务能力的评价提供数据支持。方法 以某院2019年1月1日-2021年12月31日住院病案首页数据为基础,利用病案信息统计和国家版诊断相关分组器(CN-DRGs)对住院病例99 113例进行统计和分组,从收治病种数量、住院手术数量、DRGs组数、CMI、DRGs时间指数、DRGs费用指数6个医疗服务能力指标对全院和科室数据分布情况进行纵向和横向的对比分析。结果 2021年医院出院患者前3位的主要诊断大类(MDC)依次为MDCB(神经系统疾病及功能障碍)、MDCE(呼吸系统疾病及功能障碍)、MDCF(循环系统疾病及功能障碍),占总出院比例41.13%。2019年1月1日-2021年12月31日MDCI(肌肉、骨骼疾病及功能障碍)出院占比和DRG权重逐年增长。科室综合评价得分前3位的科室为O、G、Q。CMI值由低到高后3位科室为L、N、P。时间消耗指数由高到低后3位科室为L、K、N。费用消耗指数由高到低后3位科室为P、O、N。结论 通过住院病案首页数据管理与统计分析,为医院的专科建设、提质增效、资源合理配置和精细化管理方面提供了数据支持。住院病案首页数据的6个医疗服务能力指标能在一定程度上为医院决策、管理方向提供数据支持。Objectives This paper aims to explore the analysis of medical service capability indicators based on the front page data of inpatient medical records to provide data support for the improvement of the medical service capabilities of key departments of public hospitals.Methods Based on the front page data of inpatient medical records in a hospital from January 1,2019 to December 31,2021,99113 inpatient cases were counted and grouped using medical record information statistics and the national version of the diagnosis--related grouper(CN-DRGs).Longitudinal and horizontal comparative analysis of the data distribution of the whole hospital and departments were performed from the six medical service capacity indicators of the number of admitted diseases,the number of inpatient operations,the number of DRGs groups,CMI,DRGs time index,and DRGs cost index.Results In 2021,the top three main diagnostic categories(MDC)of hospital discharged patients were MDCB(nervous system diseases and dysfunction),MDCE(respiratory system diseases and dysfunction),MDCF(circulatory system diseases and dysfunction),accounting for 41.13%of the total discharge rate.From January 1,2019 to December 31,2021,the proportion of MDCI(muscle and bone diseases and dysfunction)discharges and the weight of DRG had increased year by year.The top 3 departments with comprehensive evaluation scores were 0,G,and Q.The last 3 departments of CMI value from low to high were L,N,P.The last three departments of the time consumption index from high to low were L,K,and N.The last three departments in the cost consumption index from high to low were P,0,N.Conclusions Data management and statistical analysis of the front pages of hospital medical records provide data support for the construction of specialized departments,the improvement of quality and efficiency,the rational allocation of resources and the fine management of the hospital.The six medical service capability indicators of the first page data of inpatient medical records can provide data support for hosp
