2019—2021年江原医院住院患者的呼吸道医院感染情况分析

摘　　要：目的分析住院患者的呼吸道医院感染风险因素,为降低医院感染的发生提供依据。方法收集2019—2021年江苏省江原医院住院患者的医院感染监测数据,采用SPSS 19.0软件对呼吸道医院感染发生情况及相关风险因素进行分析,以P<0.05为差异有统计学意义。结果2019—2021年江原医院共发生医院感染89例,总感染率为0.59%;其中呼吸道医院感染53例,占总感染病例数的59.55%(53/89)。53例病例中,上呼吸道感染35例(66.04%),下呼吸道感染18例(33.96%);各年病例数分别为24、18和11例,分别占各年病例总数的75.00%(24/32)、66.67%(18/27)和36.67%(11/30);各年感染率分别为0.47%(24/5094)、0.39%(18/4599)和0.20%(11/5499),总体呈逐年下降趋势(χ^(2)_(趋势)=5.647,P=0.021);内科患者感染率从2019年的0.93%下降为2021年的0.10%,下降了89.25%。53例呼吸道医院感染者的入院基础疾病及治疗主要以甲状腺恶性肿瘤术为主(27例,50.94%),其次为弥漫性甲状腺肿(Graves病)(10例,18.87%)和2型糖尿病(7例,13.21%);共送检了20份痰液标本,仅在下呼吸道感染者痰液标本检出了病原菌,阳性率为38.89%(7/18)。结论2019—2021年江原医院住院患者的医院感染以呼吸道感染为主,且有逐年下降趋势。Objective To analyze the risk factors of respiratory nosocomial infection among inpatients,and to provide basis for reducing the incidence of nosocomial infection.Methods The monitoring data of nosocomial infection among inpatients in Jiangyuan Hospital of Jiangsu Province from 2019to 2021 were collected,and the incidence of respiratory nosocomial infection and its related risk factors were analyzed through SPSS 19.0.P 0.05 was considered to be statistically significant.Results From 2019 to 2021,a total of 89 cases of nosocomial infection occurred in Jiangyuan Hospital,with a total infection rate of 0.59%.There were 53 cases of respiratory nosocomial infection,accounting for 59.55%(53/89)of the total number of infections.Among the 53 cases,35 cases(66.04%)had an upper respiratory tract infection and 18 cases(33.96%)had a lower respiratory tract infection.The number of cases in each year was 24,18 and 11,accounting for 75.00%(24/32),66.67%(18/27)and 36.67%(11/30)of the total cases in each year,respectively.The annual infection rates were 0.47%(24/5094),0.39%(18/4599)and 0.20%(11/5499),respectively,showing a downward trend year by year(χ^(2)_(trend)=5.647,P=0.021).The infection rate of internal medicine patients decreased from 0.93%in 2019 to 0.10%in 2021 by 89.25%.Among the 53patients with respiratory nosocomial infection,the main underlying diseases and treatment at admission were thyroid cancer surgery(27 cases,50.94%),followed by diffuse goitre(10 cases,18.87%)and typeⅡdiabetes(7 cases,13.21%).A total of 20 sputum samples were detected,and pathogenic bacteria were detected only in sputum samples of patients with lower respiratory tract infection,with a positive rate of 38.89%(7/18).Conclusions From 2019 to 2021,the main nosocomial infection of hospitalized patients in Jiangyuan Hospital was respiratory tract infection,which showed a decreasing trend year by year.
