穴位埋线疗法联合八白散对肝气郁结型黄褐斑患者中医证候及皮损程度的影响

摘　　要：目的探讨穴位埋线疗法联合八白散在肝气郁结型黄褐斑患者中的运用效果。方法选取2020年1月-2021年12月我院收治的126例肝气郁结型黄褐斑患者,按随机数字表法分2组,各63例。对照组给予常规治疗,观察组加用穴位埋线疗法联合八白散治疗,持续治疗12周。对比两组临床疗效、中医证候积分、皮损程度、皮肤相关指标、生活质量及复发率。结果观察组治疗总有效率95.24%,高于对照组的84.13%,有统计学差异(P<0.05);治疗前两组中医证候积分、皮损程度、皮肤相关指标、生活质量比较,无统计学差异(P>0.05);治疗12周,观察组中医证候积分(4.69±1.08)分、皮损面积评分(0.54±0.16)分、皮损颜色评分(0.49±0.12)分、皮肤相关指标中的色素斑评分(14.13±3.05)分、皱纹评分(15.33±3.45)分、纹理评分(10.78±2.20)分、毛孔评分(17.82±2.71)分、紫质评分(20.12±3.45)分、皮肤病生活质量量表(DLQI)评分(5.24±0.78),均低于对照组的(6.30±1.56)分、(0.80±0.19)分、(0.68±0.14)分、(17.65±4.31)分、(20.13±3.79)分、(15.13±3.19)分、(20.49±3.01)分、(24.90±3.28)分、(6.45±0.82)分,差异有统计学意义(P<0.05);观察组复发率为7.94%,低于对照组的22.22%,有统计学差异(P<0.05)。结论穴位埋线疗法联合八白散能增强肝气郁结型黄褐斑治疗效果,降低中医证候积分,减轻皮损程度,并改善皮肤指标与生活质量,减少疾病复发。Objective To explore the application effect of acupoint catgut embedding therapy combined with Babai powder in patients with chloasma of liver-qi stagnation type.Methods From January 2020 to December 2021,126 patients with melasma due to stagnation of liver qi were selected and divided into two groups according to random number table,with 63 patients in each group.The control group was given routine treatment,and the observation group was treated with acupoint catgut embedding therapy combined with Babai powder for 12 weeks.The clinical efficacy,TCM syndrome score,skin lesion degree,skin related indicators,quality of life and recurrence rate were compared between the two groups.Results The total effective rate of treatment in the observation group was 95.24%,which was higher than 84.13%in the control group(P<0.05);Before treatment,there was no statistical difference between the two groups in TCM syndrome score,skin lesion degree,skin related indicators and quality of life(P>0.05);After 12 weeks of treatment,the observation group had TCM syndrome score(4.69±1.08),skin lesion area score(0.54±0.16),skin lesion color score(0.49±0.12),pigmentation spot score(14.13±3.05),wrinkle score(15.33±3.45),texture score(10.78±2.20),pore score(17.82±2.71),and purple quality score(20.12±3.45)The score of the Dermatology Quality of Life Scale(DLQI)(5.24±0.78)was lower than that of the control group(6.30±1.56),(0.80±0.19),(0.68±0.14),(17.65±4.31),(20.13±3.79),(15.13±3.19),(20.49±3.01),(24.90±3.28)and(6.45±0.82),with a statistically significant difference(P<0.05);The recurrence rate of the observation group was 7.94%,lower than that of the control group(22.22%),with statistical difference(P<0.05).Conclusion Acupoint catgut embedding therapy combined with Babai powder can enhance the therapeutic effect of melasma due to stagnation of liver-qi,reduce the integral of TCM syndromes,reduce the degree of skin lesions,improve skin indicators and quality of life,and reduce disease recurrence.
