激光汽化术与等离子电切术在良性前列腺增生患者中的应用效果对比

摘　　要：目的对比分析1470nm半导体激光汽化术与经尿道等离子双极电切术(TUPKP)治疗良性前列腺增生(BPH)的临床效果。方法选取重庆市九龙坡区人民医院2018年11月-2021年11月收治的BPH患者82例,通过随机数字表法分为TUPKP组和激光汽化术组各41例。TUPKP组行TUPKP,激光汽化术组行1470nm半导体激光汽化术,比较2组临床疗效,围术期指标,术前、术后1个月的排尿功能、生活质量评分(QOL)、前列腺症状评分(IPSS),术后并发症发生情况。结果2组总有效率、术后并发症发生率比较,差异无统计学意义(P>0.05)。激光汽化术组膀胱冲洗时间、住院时间短于TUPKP组,血钠、血红蛋白下降值少于TUPKP组(P<0.05)。与术前相比,术后1个月,2组残余尿量减少,最大尿流率加快,前列腺体积缩小,QOL评分、IPSS评分降低,但组间比较差异均无统计学意义(P>0.05)。结论1470nm半导体激光汽化术、TUPKP治疗BPH均有较好的疗效和安全性,均可减轻患者临床症状,改善排尿功能及生活质量,但1470nm半导体激光汽化术有膀胱冲洗时间、住院时间更短,血钠、血红蛋白下降值低的优势。Objective To compare and analyze the application effect of 1470 nm semiconductor laser vaporization and transurethral plasmakinetic prostatectomy(TUPKP)in the treatment of benign prostatic hyperplasia(BPH).Methods A total of 82 patients with BPH who were admitted to the Chongqing Jiulongpo District People's Hospital from November 2018 to November 2021 were selected as the research subjects,and they were divided into the TUPKP group and the laser vaporization group by random number table method,with 41 cases in each.The TUPKP group received TUPKP,and the laser vaporization group received 1470 nm semiconductor laser vaporization.The two groups were observed for 1 month after operation.The clinical efficacy 1 month after surgery,perioperative indexes,urination function,score of quality of life score(QOL),prostate symptom score(IPSS)before and 1 month after surgery,postoperative complications were compared between the two groups.Results There was no significant difference in the total effective rate 1 month after surgery and postoperative total complication rate between the laser vaporization group and the TUPKP group(P>0.05).The times of bladder flushing and hospital stay of laser vaporization group were shorter than those of TUPKP group,and the decrease values of blood sodium and hemoglobin were lower than those of TUPKP group(P<0.05).Compared with the pre-operation,1 month after the operation,the residual urine volume of the two groups decreased,the maximum urine flow rate of the two groups was accelerated,the prostate volume of the two groups decreased,the scores of QOL and IPSS of the two groups decreased(P<0.05),but there was no statistical significance between the two groups(P>0.05).Conclusion 1470 nm semiconductor laser vaporization,TUPKP treatment of BPH all had better curative effect and safety,could reduce the patients'clinical symptoms,improve patient micturition function and quality of life,but there were 1470 nm semiconductor laser vaporization had shorter times of bladder flushing and hospital stay,ser
