头高臀低患侧高截石位在输尿管镜碎石中临床应用

摘　　要：目的探讨头高臀低患侧高截石位在输尿管镜碎石取石中的临床应用效果。方法回顾性分析和平县人民医院2021年1月-2022年5月收治的49例患者临床资料,均采用头高臀低患侧高截石位行输尿管镜碎石术,观察治疗效果。结果本组患者术中均成功击碎结石,术中未发生结石上移,结石清除率100%(49/49)。平均手术时间(33.20±18.36)min;平均住院天数(3.84±1.49)d;术中无输尿管穿孔等并发症;术后随访1~3个月,结石无复发。结论头高臀低患侧高截石位较传统截石位用于输尿管镜碎石手术的优势为减少了结石上移,利于提高碎石成功率。且头高臀低患侧高截石位要求不高,只需在传统截石位上加以调整,非常适合在基层医院推行。Objective To investigate the clinical application of head-high and hip-low high lithotomy position with affected side in ureteroscopic lithotripsy(URL).Methods A retrospective study was made on 49 cases with ureteral calculi from January 2021 to May 2022.All patients underwent ureteroscopic lithotripsy in the high lithotomy position with head-high and hip-low affected side.ResultsAll of case,operation was successful.The average operative time was(33.20±18.36)min.No calculus movement occurred during the operation,and the calculus clearance rate was 100%(49/49).The average hospital stay was(3.84±1.49)d.There was no recurrence of calculus during the follow-up of 1 to 3 months after operation.Conclusion Compared with the traditional lithotomy position,the advantages of ureteroscopic lithotripsy in the head-high and hip-low high lithotomy position with affected side are as follows:It reduces the upward movement of calculus,which is beneficial to improve the success rate of calculus crushing.In addition,this position does not require high requirements,and only needs to be adjusted in the traditional lithotomy position.It is very suitable for implementation in primary hospitals and is an effective and convenient treatment method.
