摘 要:目的 比较股骨颈动力交叉钉系统(FNS)与空心加压螺钉(CCS)固定治疗股骨颈骨折的临床疗效。方法 将33例单侧股骨颈骨折患者根据手术方式不同分为FNS组(13例)和CCS组(20例)。记录两组切口长度、手术时间、术中出血量、术中透视次数、股骨近端短缩长度(PFS)、骨折愈合时间、髋关节功能Harris评分以及术后并发症发生情况。结果 患者均获得12个月随访。术中出血量和切口长度FNS组多(长)于CCS组(P<0.05)。术中透视次数、手术时间、骨折愈合时间、末次随访髋关节功能Harris评分、末次随访PFS>5 mm的例数及术后并发症发生率FNS组均优于CCS组(P<0.05)。结论 与CCS固定相比,采用FNS固定治疗股骨颈骨折具有手术时间短、术中透视次数少、内固定稳定、骨折愈合时间短、髋关节功能恢复好、术后并发症少等优点。Objective To compare the clinical efficacy of the dynamic crossing femoral neck system(FNS)and the cannulated compression screw(CCS)fixation in the treatment of femoral neck fractures.Methods The 33 patients with unilateral femoral neck fractures were divided into FNS group(13 cases)and CCS group(20 cases),according to different operation methods.These data were recorded,including the surgical incision length,operation time,intraoperative blood loss,intraoperative fluoroscopy times,proximal femoral shortening(PFS),fracture healing time,Harris score of hip function and postoperative complications.Results All patients were followed up for 12 months.The intraoperative blood loss and incision length of the FNS group were more(longer)than those of the CCS group(P<0.05).The FNS group was better than CCS group in the intraoperative fluoroscopy times,operation time,fracture healing time,Harris score of hip function at last follow-up,and the number of cases with PFS>5 mm at last follow-up,and the incidence rate of complications(P<0.05).Conclusions Compared with CCS fixation,FNS fixation for femoral neck fracture has the advantages of shorter surgical time,fewer intraoperative fluoroscopy times,rigid internal fixation,shorter fracture healing time,and better hip function,fewer postoperative complications.
