摘　　要：目的 探讨股骨近端防旋髓内钉治疗股骨转子间骨折的疗效。方法 采用股骨近端防旋髓内钉治疗36例股骨转子间骨折患者,比较不同类型的外侧壁破裂患者的手术情况及疗效。结果 患者均获得12个月随访。术中出血量10~300(101.67±76.29)ml,术后住院天数1~11(5.6±2.7)d,住院总天数2~26(9.9±4.4)d。术后1年髋关节Harris评分78~96(88.3±5.3)分。术中出血量、术后住院天数、住院总天数、术后1年髋关节Harris评分:不同类型的外侧壁破裂患者比较差异均无统计学意义(P>0.05)。结论 PFNA治疗伴有外侧壁破裂的股骨转子间骨折,疗效良好,并不需要常规重建外侧壁。Objective To investigate the effect of proximal femoral nail anti-rotation(PFNA)on the femoral intertrochanteric fracture.Methods The 36 patients with femoral intertrochanteric fractures were treated by PFNA,and the surgery conditions and efficacy of the patients with different type lateral wall fractures were compared.Results All patients were followed up for 12 months.Intraoperative blood loss was 10~300(101.67±76.29)ml,postoperative hospital stay was 1~11(5.6±2.7)d,and total hospital stay was 2~26(9.9±4.4)d.At 1 year after surgery,the Harris score of hip joint was 78~96(88.3±5.3)points.In different types of lateral wall fractures patients,there were no significant differences in intraoperative blood loss,postoperative length of hospital stay,total length of hospital stay,and Harris score of hip joint at 1 year after surgery(P>0.05).Conclusions PFNA has good effect in the treatment of femoral intertrochanteric fracture with lateral wall fracture,and it is not needed for conventional reconstruction of the lateral wall.
