妊娠中期瘢痕子宫破裂并发弥散性血管内凝血一例

摘　　要：随着剖宫产率的增加,二胎、三胎政策的放开,瘢痕子宫再次妊娠的人群也在逐渐增加,相关妊娠并发症尤其是子宫破裂的发生率逐年升高。子宫破裂是产科严重的妊娠并发症,识别不清、处理不当可致孕妇与胎儿死亡。报告1例妊娠14+2周瘢痕子宫破裂发生出血性休克、弥散性血管内凝血(disseminated intravascular coagulation,DIC)的患者,在积极输血输液、纠正休克和DIC的同时剖腹探查行剖宫取胎术,术后未发生严重并发症,好转出院。通过回顾患者妊娠期情况及子宫破裂后的治疗过程,总结经验,强化临床医生对该病的认识及熟悉相关的诊疗方案,早期识别子宫破裂,及时手术,以减少患者发生急危重症情况的可能。With the increase of caesarean section rates,and opening two-child,three-child policy,the number of pregnant women with scarred uterine is gradually increasing.The incidence of pregnancy complications,especially rupture of uterus is increasing year by year.Rupture of uterus is a serious complication of pregnancy in obstetrics,if not clearly identified,mishandled,can lead the pregnant women and fetuses to death.In this case,a pregnant woman with rupture of uterus at 14+2 w,went into hemorrhagic shock and disseminated intravascular coagulation(DIC).Exploratory laparotomy was performed while shock and DIC were corrected by transfusion and infusion,she improved and was discharged from hospital without serious complications.By reviewing the patient′s pregnancy and the treatment process,summarizing experience,strengthening clinicians′cognition of such diseases and familiarity with related diagnosis and treatment plans,we can early identify rupture of uterine and timely perform operation,in order to reduce the possibility of emergency and critical conditions for patients.
