摘　　要：目的:观察柴胡疏肝散联合俞募配穴法治疗围绝经期抑郁症的临床疗效。方法:选择符合纳入标准的60例围绝经期抑郁症患者,依据随机数字表随机分为对照组和治疗组,每组各30例。对照组口服盐酸文拉法辛缓释片,75~225 mg/次,1次/d;治疗组口服柴胡疏肝散联合俞募配穴法(选穴:心俞、巨阙、肝俞、期门、肾俞、京门、脾俞、章门、百会、四神聪、少海、内关、三阴交、膻中、太冲、关元及安眠穴)进行治疗;两组均连续治疗6周。比较两组患者治疗前后HAMD、改良Kupperman评分和中医证候积分的变化情况,并比较临床疗效。结果:治疗后治疗组总有效率为96.67%(29/30),对照组总有效率为70%(21/30),两组比较有统计学意义(P<0.05);与治疗前相比,治疗后两组患者HAMD评分、改良Kupperman评分均明显降低,且治疗后治疗组HAMD、改良Kupperman评分改善情况明显优于对照组(P<0.05);两组患者治疗后中医证候主症积分、次症积分及总积分均较治疗前明显降低(P<0.05),且治疗后治疗组中医证候主症积分、次症积分及总积分改善情况明显优于对照组(P<0.05)。结论:柴胡疏肝散联合俞募配穴法针刺治疗可明显改善围绝经期抑郁症患者的情绪障碍及临床相关症状,临床疗效显著。Objective:To observe the clinical efficacy of Chaihu Shugan Powder combined with Shu-Mu points compatibility in treatment of perimenopausal depression.Methods:60 patients with perimenopausal depression were randomly divided into the control group and the treatment group,with 30 patients in each group.The control group was treated with Venlafaxine Hydrochloride Sustained-Release Tablet,75 mg-225 mg each time,once a day;the treatment group was treated with Chaihu Shugan Powder combined with Shu-Mu points compatibility,the points chosen included Xinshu(BL15),Juque(RN14),Ganshu(BL18),Qimen(LR14),Shenshu(BL23),Jingmen(GB25),Pishu(BL20),Zhangmen(LR13),Baihui(DU20),Sishencong(EX-HN1),Shaohai(HT3),Neiguan(PC6),Sanyinjiao(SP6),Danzhong(RN17),Taichong(LR3),Guanyuan(RN4)and Anmian point.Both groups were treated continuously for 6 weeks.HAMD score,modified Kupperman score and TCM symptoms scores were compared between before and after the treatment in the two groups.Clinical efficacy was also compared between the two groups.Results:The total effective rate of the treatment group was 96.67%(29/30),which was significantly higher than 70%(21/30)of the control group(P<0.05).Compared with those before the treatment,HAMD score and modified Kupperman score were significantly decreased after the treatment in the two groups,and the improvements of HAMD score and modified Kupperman score were more significant in the treatment group than those in the control group(P<0.05).The main symptom score,the secondary symptom score and the total score of TCM symptoms were significantly decreased after the treatment compared with those before the treatment in the two groups(P<0.05),of which the improvements in the treatment group were more significant than those in the control group(P<0.05).Conclusion:Chaihu Shugan Powder combined with Shu-Mu points compatibility can significantly improve mood disorders and clinical symptoms of perimenopausal depression with significant clinical effect.The therapy is worthy of popularization and application.
