摘　　要：目的促进甲钴胺注射液的临床合理应用。方法对1例静脉用甲钴胺注射液致过敏性休克案例进行分析,结合文献进一步探讨甲钴胺注射液致过敏性休克的临床特点。结果患者在最近一次使用甲钴胺注射液30 min内发生过敏性休克,属速发型过敏反应,发生原因可能与药品因素、个体差异、给药方式、用药剂量等有关。结论甲钴胺注射液可引起过敏性休克,但其药品说明书未予记载和警示。甲钴胺注射液宜采用肌肉注射或静脉注射方式给药,建议老年患者酌情减量使用;加强用药监护,避免发生严重药品不良反应。Objective To promote the rational clinical use of Mecobalamin Injection.Methods A case of anaphylactic shock induced by Mecobalamin Injection was analyzed,and the clinical and characteristics of anaphylactic shock induced by Mecobalamin Injection were further investigated based on literature analysis.Results Anaphylactic shock occurred within 30 min of the latest administration of Mecobalamin Injection,which was a type of immediate allergic reaction.The occurrence of anaphylactic shock may be related to drug factors,individual differences,administration methods and dosage.Conclusion Mecobalamin Injection may induce anaphylactic shock,but its package inserts do not contain a warning.Mecobalamin Injection should be administered intramuscularly or intravenously,and it is recommended that elderly patients should reduce the dosage as appropriate.Medication monitoring should be strengthened to avoid the occurrence of severe adverse drug reaction.
