摘  要:目的观察“四四固本颗粒”对脾肾阳虚型IBS-D模型大鼠肠道菌群及其代谢产物短链脂肪酸(SCFAs)的影响,从“肠道菌群”角度揭示“四四固本颗粒”对IBS-D防治作用及其作用机制。方法将60只SPF级SD雄性大鼠随机分为6组,采用番泻叶灌胃+避水应激方法建立脾肾阳虚证IBS-D大鼠模型,造模后各组大鼠给予相应药物,采用高通量测序检测大鼠肠道菌群,使用气相色谱-质谱联用(GC-MS)检测肠道内容物菌群关键代谢产物短链脂肪酸表达。结果与模型组比较,SSGB组大鼠粪便Bristol评分均显著降低(P<0.05);阳性药组和SSGB-H组AWR显著降低(P<0.05);SSGB-H/M/L组D-木糖、ACTH、CORT、尿17-OHCS含量均有不同程度增加;SSGB组Shannon指数显著增加(P<0.05),Simpson指数显著降低(P<0.05);SSGB-H组厚壁菌门和变形菌门丰度都显著降低(P<0.05),拟杆菌门丰度显著升高(P<0.05),而SSGB-M/L组放线菌门丰度显著升高(P<0.05);SSGB组在SCFAs总酸、丙酸、乙酸都显著低于模型组(P<0.05)。结论SSGB能够明显改善大鼠脾肾阳虚症状,降低大鼠内脏敏感性,增加IBS-D大鼠肠道菌群多样性,有效逆转IBS-D大鼠肠道菌紊乱,提高益生菌丰度,降低致病菌丰度,促进肠道菌群代谢产物短链脂肪酸吸收,进而调节肠道菌群。Objective To observe the effect of Sisi Guben Granule(四四固本颗粒)on the intestinal flora and its metabolites of short-chain fatty acids(SCFAs)in diarrhea-predominant irritable bowel syndrome(IBS-D)model rats with deficiency of spleen and kidney Yang,and to reveal the preventive effect and therapeutic mechanism of Sisi Guben Granule on IBS-D from the perspective of intestinal flora.Methods Dividing 60 male SD rats of SPF into 6 groups,to establish the models of IBS-D rats which were deficiency of spleen and kidney Yang by gavage of senna and water avoidance stress.After the model were established,the rats in each group were given corresponding drugs,then to detect the intestinal flora of rats by the high-throughput sequencing,and detect the expression of the key metabolites of intestinal flora that is SCFAs by gas chromatography-mass spectrometry(GC-MS).Results Compared with the model group,the Bristol score of rats in SSGB group decreased significantly(P<0.05);the AWR decreased significantly in positive drug group and SSGB-H group(P<0.05);The contents of D-xylose,ACTH,CORT and urinary 17-OHCS increased in SSGB-H/M/L group;The Shannon index increased significantly in SSGB group(P<0.05),the Simpson index decreased significantly(P<0.05),and the abundance of Firmicutes and Proteus decreased significantly in SSGB-H group(P<0.05),the abundance of Bacteroidetes increased significantly(P<0.05),while the abundance of actinomycetes in SSGB-M/L group increased significantly(P<0.05);The total acid,propionic acid and acetic acid in SCFAs in SSGB group were significantly lower than those in model group(P<0.05).Conclusion SSGB can significantly improve the symptoms of deficiency of spleen and kidney Yang in rats,can reduce visceral sensitivity in rats and increase the diversity of intestinal flora in IBS-D rats,it also can effectively reverse the disorder of intestinal bacteria in IBS-D rats,to increase the abundance of probiotics,and reduce the abundance of pathogenic bacteria,promote the absorption of metabolites of i
