基于络病理论治疗间质性肺病临床经验探析

摘　　要：间质性肺病是一组肺部弥漫性疾病的总称,临床发病率高、治愈率低。文章对络病理论与间质性肺病关系进行了讨论,阐明两者具有生理结构认识的相似性和病理变化过程的趋同性之特点。基于黑龙江省名中医张福利教授从络病论治间质性肺病的临床经验,总结出其根据个体施治,以“通调肺络,造化荣络”立法,遵循“气病及血-由经入络”病机路径的独特治疗手段。根据疾病侵犯气血层次的变化,为祛除“湿热”“血瘀”“痰凝”因素导致的肺络痹阻之势,以清络、透络、养络的证治理念强调气血同治、多层同调的临床治疗手段应对此缠绵疾病,现取得良好的疗效,附验案一则以佐证。并为新型冠状病毒感染恢复期患者肺间质纤维化改变的防治提供思考。Interstitial pneumonia is a general term for a group of diffuse lung diseases,which has the characteristics of high incidence and low cure rate.This paper discusses the relationship between collateral disease theory and interstitial lung disease,and expounds that they have the characteristics of similarity in physiological structure and convergence in pathological change process.Analysis of Professor ZHANG Fuli's experience in treating interstitial pneumonia from the theory of collateral disease,this paper summarizes Professor ZHANG's unique treatment method according to the patients'physical condition,which follows the pathogenesis path of“Qi disease and blood-entering the collaterals from meridians”with the basic method of“dredging and nourishing the lung collaterals”.According to the changes of the level of Qi and blood invaded by the disease,first remove the pulmonary obstruction caused by the factors of“damp heat”“blood stasis”and“phlegm coagulation”,and emphasize the clinical treatment of the lingering disease with the concept of clearing,penetrating and nourishing collaterals.These therapies have achieved good results.An clinical case is attached to prove it.
