基于异病同治探讨器官移植与免疫型复发性流产治疗思路

摘　　要：通过分析器官移植与免疫型复发性流产的西医病理机制和中医病因病机,认为二者存在诸多相同点,在中医上则均以肾虚为本,兼有脾虚,以瘀血、痰浊等为标,形成本虚标实之证。二者具有异病同治之理论基础,故治疗方法可相互借鉴,主要有补肾安胎法、健脾祛湿法、活血化瘀法,在药物上可选择运用现代药理研究表明具有免疫调节作用的药物,如冬虫夏草、丹参、山茱萸、雷公藤等。同时,在重视“证同”之外,还需注意“病异”之处,针对二者的疾病特点及临床表现,宜在治法和用药上加以区别。尤其是妊娠用药的特殊性,宜考虑部分用药的安全性。By analyzing the pathological mechanism of western medicine and the etiology and pathogenesis of Chinese medicine of organ transplantation and immunologic recurrent miscarriage, it is believed that there are many similarities. In Chinese medicine,both of them take kidney deficiency as the root cause, complicated with spleen deficiency, and take blood stasis and phlegm turbidity as the symptoms, thus forming the syndrome of deficiency in the origin and excess in the superficiality. They share the same theoretical basis of the same treatment for different diseases,so their treatment methods can be learned from each other, mainly including kidney-tonifying and abortion-preventing method,spleen-fortifying and dampness-dispelling method,and blood-activating and stasis-resolving method. In terms of medication, medicines with immunomodulatory effects proven by modern pharmacological studies can be selected for application,such as Cordyceps,Salviae Miltiorrhizae Radix et Rhizoma,Corni Fructus, Tripterygii Wilfordii Radix, etc. At the same time, in addition to the “same syndrome”,“differences between diseases” should also be paid attention to. According to their disease characteristics and clinical manifestations,it is appropriate to distinguish their treatment and medication. Especially for the particularity of medication use in pregnancy,the safety of some medicines should be considered.
