抗精子抗体、抗子宫内膜抗体、抗心磷脂抗体、抗核抗体检测对复发性流产的诊断价值分析

摘　　要：目的:探讨抗精子抗体(AsAb)、抗子宫内膜抗体(EMAb)、抗心磷脂抗体(ACA)、抗核抗体(ANA)检测对复发性流产(RSA)的诊断价值。方法:选取2018年6月—2019年5月东莞市人民医院收治的80例RSA患者作为观察组,选取同期入院体检的80例已生育健康女性作为对照组。采取酶联免疫吸附法测定两组AsAb、EMAb、ACA水平,以间接免疫荧光法测定ANA水平。比较两组各指标的阳性率,并对AsAb、EMAb、ACA、ANA与RSA的相关性进行分析。结果:观察组AsAb、EMAb、ACA、ANA阳性率均高于对照组,差异有统计学意义(P<0.05)。AsAb、EMAb、ACA、ANA指标联合检测的阳性率高于各单项指标检测阳性率,差异有统计学意义(P<0.001)。AsAb、EMAb、ACA、ANA阳性率与RSA均呈正相关。结论:AsAb、EMAb、ACA、ANA阳性率与RSA有相关性,可用于RSA的病因筛查,4项指标联合检测可提高RSA筛查的敏感性,诊断价值显著,能为病因分析和免疫治疗提供依据。Objective:To investigate the diagnostic value of antisperm antibody(AsAb),antiendometrial antibody(EMAb),anticardiolipin antibody(ACA)and antinuclear antibody(ANA)detection in recurrent spontaneous abortion(RSA).Methods:From June 2018 to May 2019,80 RSA patients admitted to Dongguan People's Hospital were selected as the observation group,and 80 healthy women who had given birth and received physical examination in this hospital during the same time period were selected as the control group.The levels of AsAb,EMAb and ACA in the two groups were determined by enzyme-linked immunosorbent method,and the ANA level was measured by indirect immunofluorescence.The positive rates of each index in the two groups were compared,and the correlations between AsAb,EMAb,ACA,ANA and RSA were analyzed.Results:The positive rates of AsAb,EMAb,ACA and ANA in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.05).The positive rate of AsAb,EMAb,ACA and ANA indexes in the combined detection was higher than that of each single index detection,and the difference was statistically significant(P<0.001).AsAb,EMAb,ACA,ANA positive rates were positively correlated with RSA.Conclusion:The positive rates of AsAb,EMAb,ACA and ANA are correlated with RSA,which can be used for etiological screening of RSA,and the combined detection of the four indexes can increase the sensitivity of RSA screening,and the diagnostic value is significant,which can provide a basis for etiology analysis and immunotherapy.
