摘　　要：目的探讨儿童传染性单核细胞增多症(IM)患儿EB病毒(EBV)-DNA载量与异型淋巴细胞百分率及T淋巴细胞亚群的相关性。方法选取2019年6月至2022年1月郑州大学附属儿童医院收治的40例IM患儿作为研究组,另选取同期健康体检的40例健康儿童作为对照组。比较两组白细胞计数、EBV-DNA载量阳性率、异型淋巴细胞百分率及T淋巴细胞亚群水平,分析EBV-DNA载量与异型淋巴细胞百分率及T淋巴细胞亚群水平的相关性。结果研究组外周血白细胞计数、异型淋巴细胞百分率、EBV-DNA载量阳性率均高于对照组,差异均有统计学意义(P<0.05);研究组CD3^(+)T淋巴细胞水平、CD8^(+)T淋巴细胞水平均高于对照组,CD4^(+)T淋巴细胞水平和CD4^(+)T淋巴细胞/CD8^(+)T淋巴细胞均低于对照组,差异均有统计学意义(P<0.05)。IM患儿EBV-DNA载量与异型淋巴细胞百分率、CD3^(+)T淋巴细胞水平、CD8^(+)T淋巴细胞水平均呈正相关(r=0.447、0.567、0.603,P<0.05);EBV-DNA载量与CD4^(+)T淋巴细胞水平、CD4^(+)T淋巴细胞/CD8^(+)T淋巴细胞均呈负相关(r=-0.268、-0.407,P<0.05)。结论IM患儿EBV-DNA载量阳性率、异型淋巴细胞百分率及T淋巴细胞亚群水平均出现明显异常,并且EBV-DNA载量与CD3^(+)T淋巴细胞水平、CD8^(+)T淋巴细胞水平及CD4^(+)T淋巴细胞/CD8^(+)T淋巴细胞均有明显相关性。Objective To investigate the correlation between EB virus(EBV)-DNA load and allogeneic lymphocyte ratio and T-lymphocyte subpopulation in pediatric patients with infectious mononucleosis(IM).Methods Retrospective analysis of 40 children with IM in children treated at the Children′s Hospital of Zhengzhou University from June 2019 to January 2022 was conducted as the study group,and 40 normal children with healthy physical examination during the same period were selected as the control group.The differences in total leukocyte count,EBV-DNA load,heterozygous lymphocyte ratio and T lymphocyte subpopulation levels between the two groups were compared,and the correlation between EBV-DNA load and heterozygous lymphocyte ratio and T lymphocyte subpopulation was analyzed.Results The percentage of peripheral blood allogeneic lymphocytes and EBV-DNA load were higher in the study group than in the control group,and the differences were statistically significant(P<0.05).The T lymphocyte subsets CD3^(+)and CD8^(+)T cells were elevated and the CD4^(+)T cells and CD4^(+)T cell/CD8^(+)T cell ratio were decreased in the study group,and the differences were statistically significant(P<0.05)compared with the control group.The IM children′EBV-DNA load was positively correlated with the allogeneic lymphocyte ratio(r=0.447,0.567,0.603,P<0.05),and it was positively correlated with the CD8^(+)T lymphocyte ratio,and negatively correlated with the CD4^(+)T cells/CD8^(+)T cell ratio(r=-0.268,-0.407,P<0.05).Conclusion Children with IM showed significant abnormalities in EBV-DNA load,allogeneic lymphocyte ratio and T lymphocyte subsets,and EBV-DNA load was significantly correlated with CD3^(+),CD8^(+)T cells and CD4^(+)/CD8^(+)T cell ratio.
