摘　　要：目的分析山东省MSM对HIV暴露前预防(PrEP)的使用意愿及影响因素。方法在山东省5个市,依托社会组织通过滚雪球方法招募年满18周岁且有过同性肛交性行为的男性作为调查对象。采用自行设计的调查问卷面对面收集调查对象的社会人口学特征、艾滋病相关行为学特征、PrEP使用意愿状况等信息。采用χ2检验和多因素Logistic回归模型分析PrEP使用意愿及相关影响因素。结果共纳入调查对象2537名,愿意使用PrEP的比例为58.6%(1487/2537)。不同年龄、婚姻状况、月收入水平、户籍地、寻找性伴方式及性行为角色特征的MSM其PrEP使用意愿差异有统计学意义。多因素分析显示,与年龄≥45岁、离异或丧偶、月收入水平<5000元、本省户籍、线下寻找性伴、性行为角色是插入方的MSM比较,年龄≤24岁(OR=1.8,95%CI:1.2~2.7),未婚(OR=2.4,95%CI:1.8~3.2)、在婚或同居(OR=2.3,95%CI:1.8~3.2),月收入水平≥5000元(OR=1.4,95%CI:1.1~1.7),外省户籍(OR=1.6,95%CI:1.2~2.3),线上寻找性伴(OR=1.4,95%CI:1.2~1.7)及性行为角色是被插入方(OR=1.5,95%CI:1.1~1.9)或两者兼有(OR=2.0,95%CI:1.6~2.6)的MSM其PrEP使用意愿相对更高。结论山东省MSM整体PrEP使用意愿较低,需要针对高年龄段、离异或丧偶、月收入水平低、本省户籍地、线下寻找性伴及性行为角色是插入方的MSM加强宣传教育,提高PrEP使用意愿。Objective To analyze the willingness and influencing factors of pre-exposure prophylaxis(PrEP)among men who have sex with men(MSM).Methods The snowball sampling method was used to recruit MSM over 18 years who have had male anal sex in the last year in 5 cities of Shandong Province.Face-to-face questionnaire interviews were conducted to collect the information,including demographic characteristics,AIDS-related behavior characteristics,and willingness to use PrEP.The chi-square test and binary logistic regression mode were used to assess factors associated with willingness of PrEP usage.Results 2537 MSM were involved in the study and 58.6%(1487)were willing to use PrEP.However,there was a significantly different in willingness to use PrEP among different groups of age,marital status,monthly income,household registration,sexual partners,and sexual role.Compared with aged≥45years,divorced or widowed,with monthly income<5000yuan,registered residence in Shandong province,the offline search for sexual partners,role for being inserted in anal intercourse,MSM were more likely to use PrEP with age≤24years(OR=1.8,95%CI:1.2-2.7),the unmarried(OR=2.4,95%CI:1.8-3.2),the married or cohabiting(OR=2.3,95%CI:1.8-3.2),the monthly income≥5000 yuan(OR=1.4,95%CI:1.1-1.7),registered residence in other provinces(OR=1.6,95%CI:1.2-2.3),the MSM who looked for partners online(OR=1.4,95%CI:1.2-1.7),the inserted sex role(OR=1.5,95%CI:1.1-1.9)or the mixed-sex role(OR=2.0,95%CI:1.6-2.6).Conclusions The willingness of Pr EP among MSM was low in Shandong province.We should publicize knowledge on Pr EP to promote the use of Pr EP among MSM,especially for those who are older,divorced or widowed,have lower monthly income,with residency in Shandong province,seek sexual partners offline,or with sex roles of being insert in anal intercourse.
