针刺腰痛穴联合活络效灵汤治疗气血瘀滞证急性腰扭伤患者的临床研究

摘　　要：目的:探讨针刺腰痛穴联合活络效灵汤治疗气血瘀滞证急性腰扭伤患者的临床疗效。方法:收集2020年5月-2022年4月江西省萍乡市中医院骨科门诊诊治的气血瘀滞证急性腰扭伤患者60例,按随机数表法分为对照组和观察组,每组各30例。对照组口服双氯芬酸钠胶囊治疗,2次/d,50 mg/次。观察组给予针刺腰痛穴联合活络效灵汤治疗。5 d为1个疗程,治疗2个疗程。观察疗效、疼痛评分、日本骨科协会(Japanese Orthopaedic Association,JOA)评分、腰椎主动前屈优良情况以及腰椎主动后屈优良情况。结果:观察组干预后疼痛视觉模拟标尺评分(visual analogue scale,VAS)低于干预前和对照组(P<0.05),JOA评分则高于干预前和对照组(P<0.05)。观察组总有效率为93.33%(28/30),高于对照组的60.00%(18/30,P<0.05)。观察组腰椎主动前屈、腰椎主动后屈的优良率均高于对照组(P<0.05)。结论:针刺腰痛穴联合活络效灵汤治疗气血瘀滞证急性腰扭伤患者的临床疗效明显,可以改善患者疼痛评分、JOA评分,提高患者的腰椎主动前屈、腰椎主动后屈优良率。Objective:To explore the clinical efficacy of acupuncture at Yaotong point combined with Huoluo xiaoling decoction in the treatment of acute lumbar sprain with qi and blood stasis syndrome.Methods:From May 2020 to April 2022,60 patients with acute lumbar sprain in the Outpatient Department of Orthopedics of Pingxiang Hospital of Traditional Chinese Medicine in Jiangxi Province were collected and divided into a control group and an observation group with 30 patients in each group according to the random number table.The control group took diclofenac sodium capsule twice a day,50 mg/time.The observation group was treated with acupuncture of Yaotong point combined with Huoluo xiaoling decoction.Five days was a course of treatment,and the treatment was 2 courses.The curative effect,pain score,Japanese Orthopaedic Association(JOA)scores,excellent condition of lumbar active flexion forward and excellent condition of lumbar active flexion backward were observed.Results:The visual analogue scale(VAS)score in the observation group after intervention was lower than that the before intervention and in the control group(P<0.05);the JOA score in the observation group after intervention was higher than that the before intervention and in the control group(P<0.05);the total effective rate in the observation group was 93.33%(28/30),and higher than that in the control group 60.00%(18/30,P<0.05);the excellent and good rate of lumbar active flexion forward and lumbar active flexion backward in the observation group were higher than those in the control group(P<0.05).Conclusion:The clinical effect of acupuncture at Yaotong point combined with Huoluo xiaoling decoction in the treatment of acute lumbar sprain patients with qi blood stasis syndrome is obvious,which can improve the pain score and JOA score of the patients,and improve the excellent and good rate of lumbar active flexion forward and lumbar active flexion backward.
