糠酸莫米松喷鼻剂联合鼻内镜手术对治疗慢性鼻窦炎患者的意义

摘　　要：目的:探讨糠酸莫米松喷鼻剂联合鼻内镜手术对治疗慢性鼻窦炎患者的意义。方法:收集2021年4月至2022年4月择期接受鼻内镜手术治疗的慢性鼻窦炎患者58例,用随机数字表法分为观察组和对照组各29例。对照组接受鼻窦内镜手术治疗,观察组接受糠酸莫米松喷鼻剂联合鼻内镜手术治疗,并进行3个月的随访。随访结束时比较两组治疗效果以及治疗前后症状评分、鼻黏液纤毛清除功能、血清炎性因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、IL-8]含量和生活质量的变化情况。结果:观察组治疗总有效率为96.55%(1/29),高于对照组的79.31%(6/29,P<0.05)。观察组治疗后鼻塞评分、流涕评分、头痛评分均低于对照组(P<0.05);黏液纤毛清除速度高于对照组,糖精清除时间短于对照组(P<0.05);血清TNF-α、IL-6、IL-8含量均低于对照组(P<0.05);SNOT-20评分低于对照组(P<0.05)。结论:糠酸莫米松喷鼻剂联合鼻内镜手术治疗慢性鼻窦炎疗效显著,能够有效改善患者症状、鼻黏液纤毛清除功能和炎症状态,提高生活质量,值得临床推荐。Objective:To investigate the significance of mometasone furoate nasal spray combined with endoscopic sinus surgery in the treatment of chronic sinusitis.Methods:From April 2021 to April 2022,58 patients with chronic sinusitis who were selected to receive endoscopic sinus surgery in our hospital were collected,and randomized number table was used to divide the patients into an observation group and a control group with 29 patients in each group.The control group received endoscopic sinus surgery alone,while the observation group received mometasone furoate nasal spray combined with endoscopic sinus surgery,and the two groups of patients were followed up for 3 months.At the end of the follow-up the treatment effect,symptom score before and after treatment,nasal mucociliary clearance function,and serum inflammatory factor[tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),interleukin-8(IL-8)]test results,and changes in quality of life were compared.Results:The total effective rate in the observation group was 96.55%(1/29),which was higher than that in the control group 79.31%(6/29,P<0.05).After treatment,the scores of nasal congestion,runny nose and headache in the observation group were lower than those in the control group(P<0.05);the mucociliary clearance speed in the observation group was higher than that in the control group,and the saccharin clearance time was shorter than that in the control group(P<0.05);the contents of serum TNF-α,IL-6 and IL-8 were lower than those in the control group(P<0.05);the SNOT-20 score in the observation group was lower than that in the control group(P<0.05).Conclusion:The combination of mometasone furoate nasal spray and endoscopic sinus surgery is effective in the treatment of chronic sinusitis,can effectively improve the symptoms,nasal mucociliary clearance function and inflammatory status of patients,and improve their quality of life,which is worthy of clinical recommendation.
