来氟米特联合糖皮质激素治疗IgG4相关性涎腺炎的临床疗效

摘　　要：目的观察来氟米特联合糖皮质激素治疗IgG4相关性涎腺炎的临床疗效。方法选取2019年1月—2021年1月福建医科大学附属漳州市医院收治的IgG4相关性涎腺炎患者40例,依照随机数字表法分为观察组和对照组,各20例。2组患者均接受糖皮质激素治疗,观察组患者在此基础上加用来氟米特治疗,2组均以1个月为1个疗程,共治疗8个疗程。比较2组患者治疗效果,治疗前、治疗3个月及8个月相关指标[C反应蛋白(CRP)、红细胞沉降率(ESR)、免疫球蛋白G(IgG)]变化及不良反应。结果观察组患者治疗总有效率为95.00%,高于对照组的70.00%(χ^(2)=4.329,P=0.037);治疗3个月、8个月,观察组患者CRP、ESR及IgG水平均低于同期对照组(P<0.05或P<0.01);观察组与对照组不良反应总发生率(15.00%vs.10.00%)比较差异无统计学意义(χ^(2)=0.229,P=0.633)。结论来氟米特联合糖皮质激素治疗IgG4相关性涎腺炎的临床疗效肯定,能有效改善相关指标水平,且用药安全度较高,值得临床推广应用。Objective To observe the clinical effect of leflunomide combined with glucocorticoid in the treatment of IgG4 associated sialadenitis.Methods Selected 40 patients with IgG4 associated sialadenitis admitted to Zhangzhou Hospital affiliated to Fujian Medical University from January 2019 to January 2021.According to the random number table method,they were divided into observation group and control group,each with 20 cases.The patients in both groups received glucocorticoid treatment,and the patients in the observation group were treated with leflunomide on this basis.Both groups took 1 month as a course of treatment,with a total of 8 courses of treatment.Compared the treatment effect of the two groups of patients,the changes of relevant indicators[C-reactive protein(CRP),erythrocyte sedimentation rate(ESR),immunoglobulin G(IgG)]before treatment,3 months and 8 months after treatment,and the occurrence of adverse reactions.Results The total effective rate of treatment in the observation group was 95.00%,higher than 70.00%in the control group(χ^(2)=4.329,P=0.037).After 3 and 8 months of treatment,the levels of CRP,ESR and IgG in the observation group were lower than those in the control group(P<0.05 or P<0.01).There was no statistically significant difference in the total incidence of adverse reactions between the observation group and the control group(15.00%vs.10.00%,χ^(2)=0.229,P=0.633).Conclusion Leflunomide combined with glucocorticoid in the treatment of IgG4 associated sialadenitis has a positive clinical effect.It can effectively improve the relevant indicators,and the drug safety is high,which is worthy of clinical application.
