摘　　要：目的探讨药学干预在儿童医院门急诊超常使用阿昔洛韦管理中的作用。方法通过开展阿昔洛韦合理使用专题培训,拟定有关阿昔洛韦合理用药共识,制定阿昔洛韦初步评价标准并开展专项处方点评等一系列药学干预措施,回顾性分析干预前及干预后门急诊阿昔洛韦使用量、使用合理性等相关数据。结果经过开展综合药学干预措施,深圳市儿童医院门急诊注射用阿昔洛韦使用量由2015年的9113人次降低至2018年的887人次,阿昔洛韦片剂使用量由2015年的2469人次降低至2018年的1599人次。开展阿昔洛韦专项处方点评,Ridit分析示2015`2018年的平均R值逐渐降低,由0.6356逐渐降低至0.5421、0.4293、0.3929(P<0.01),处方合理率由2015年6.67%提高至2018年36.33%(P<0.01),使用合理性显著改善。结论药学干预在儿童医院药物合理使用中发挥重要作用。OBJECTIVE To investigate the impact of pharmacist interventions on the abnormal use of acyclovir in a Chinese tertiary children hospital.METHODS A pre-to-post intervention study of acyclovir use was performed in the outpatient emergency of the Shenzhen Children's Hospital.Three methods of pharmaceutical interventions were mainly adopted,including topic-specific training on the rational use of acyclovir,the pharmacy department taking the lead in drawing up summary of current evidence and policies on the use of antiviral drugs in hospitals,establishing specific evaluation standards for acyclovir use and performing a retrospective prescription review.RESULTS SAfter carrying out comprehensive pharmaceutical intervention measures,the use of acyclovir for injection in our hospital's outpatient emergency department was reduced from 9113 in 2015 to 887 in 2018,and the use of acyclovir tablets was reduced from 2469 in 2015 to 1599 in 2018.A special prescription review of acyclovir was carried out,and Ridit analysis showed that the reasonable distribution of acyclovir used from 2015 to 2018 was significant(P<0.01).Compared with 2015,the average R value from 2016 to 2018 had gradually decreased,from 0.6356 to 0.5421,0.4293,and 0.3929(P<0.01),the reasonable rate of prescriptions increased from 6.67%in 2015 to 36.33%in 2018(P<0.01),so the rationality of acyclovir use had been significantly improved.CONCLUSION Pharmaceutical intervention plays an important role in the rational use of drugs in a children's hospital.
