真空负压装置联合萆菟汤药浴治疗Ⅲ型前列腺炎合并勃起功能障碍的疗效观察

摘　　要：目的探讨真空负压装置(VED)联合萆菟汤药浴治疗Ⅲ型前列腺炎合并勃起功能障碍(ED)患者的临床疗效,以及前列腺小体外泄蛋白(PSEP)对于Ⅲ型前列腺炎的临床价值。方法选取2021年3月至9月黑龙江省中医药科学院男科门诊收治的80例Ⅲ型前列腺炎合并ED患者作为研究对象。采用随机数字表法分为观察组(n=40)和对照组(n=40)。两组均使用VED配合服用前列舒通胶囊治疗,观察组VED内注入萆菟汤药浴液,对照组VED注入生理盐水,治疗周期均为4周。严格记录治疗前后的美国国立卫生研究院慢性前列炎症状指数(NIH-CPSI)、国际勃起功能指数-5(IIEF-5)和尿液PSEP水平。结果与治疗前比较,两组治疗后的NIH-CPSI评分、IIEF-5评分、PSEP水平均有所改善,且观察组优于对照组,差异具有统计学意义(P<0.05);NIH-CPSI评分与PSEP水平呈中度正相关(P<0.001,R=0.539)。结论VED联合萆菟汤药浴治疗Ⅲ型前列腺炎合并ED的疗效优于单独使用VED,患者症状得以改善,勃起功能亦有所提高。PSEP水平对于Ⅲ型前列腺炎的诊断及病情严重程度的评估具有一定参考价值。Objective To investigate the clinical efficacy of vacuum erectile device(VED)combined with Bitu decoction bath in the treatment of typeⅢprostatitis complicated with erectile dysfunction(ED),and the clinical value of prostatic exosomal protein(PSEP)for typeⅢprostatitis.Methods A total of 80 patients with typeⅢprostatitis complicated with ED admitted to Department of Andrology Outpatient of Heilongjiang Academy of Traditional Chinese Medicine from March 2021 to September 2021 were selected as the study objects.The patients were divided into observation group(n=40)and control group(n=40)by adopting a random number table method.Both groups were treated by VED combined with Qianlie-Shutong capsule.The observation group′s VED was injected with Bitu decoction,while the control group was injected with normal saline.Both of the treatment course was 4 weeks.The levels of the national institutes of health chronic prostatitis symptom index(NIH-CPSI),international index of erectile function-5(IIEF-5)and PSEP were recorded and analyzed before and after treatment.Results Compared with the before treatment,the NIH-CPSI scores,IIEF-5 scores and PSEP levels improved in both two groups after treatment,and those in observation group was better than control group,the differences were all significant(P<0.05);NIH-CPSI scores was moderately positively correlated with PSEP levels(P<0.001,R=0.539).Conclusions The efficacy of VED combined with Bitu decoction bath in the treatment of typeⅢprostatitis combined with ED was better than that of VED only.The symptoms and erectile function were improved.PSEP level has certain reference value for diagnosis and assessment of the severity of the condition for typeⅢprostatitis.
