2019年—2021年西安市西咸新区婚前检查梅毒感染情况及其危险因素分析

摘　　要：目的探讨西安市西咸新区婚前检查梅毒感染情况及其危险因素分析。方法选取2019年11月至2021年12月西安市西咸新区进行婚前检查的14579例体检者作为研究对象,统计婚前检查者梅毒感染情况和基线资料,并采用单因素和多因素回归分析梅毒感染的危险因素。结果14579例婚前检查者中,梅毒检出率为0.47%(68/14579),其中一期梅毒38例(55.88%),二期梅毒13例(19.12%),隐性梅毒16例(23.53%);采用二元Logistic分析,性伴数>2个(OR=3.543,P=0.000)、从未/偶尔使用安全套(OR=4.341,P=0.000)、有男男性行为(OR=8.014,P=0.000)、有醉酒性行为(OR=2.504,P=0.004)、纹身(OR=3.309,P=0.000)、商业性行为(OR=3.197,P=0.000)均为影响婚前检查者梅毒感染的独立危险因素。结论性伴数>2个、从未/偶尔使用安全套、男男性行为、醉酒性行为、纹身、商业性行为均为影响西安市西咸新区婚前检查梅毒感染的独立危险因素,临床可从这几个方面入手,制定预防性措施。Objective To explore syphilis infection and risk factors in premarital examinations in Xixian New District,Xi′an City.Methods A total of 14,579 cases undergoing premarital examination in Xixian New District,Xi′an City were enrolled as the research objects between November 2019 and December 2021.The situation of syphilis infection and general data were statistically analyzed.The risk factors of syphilis infection were analyzed by univariate and multivariate regression analysis.Results In the 14,579 controls undergoing premarital examination,detection rate of syphilis infection was 0.47%(68/14,579),including 38 cases(55.88%)with primary syphilis,13 cases(19.12%)with secondary syphilis and 16 cases(23.53%)with latent syphilis.Binary Logistic analysis showed that number of sexual partners>2(OR=3.543,P=0.000),no/occasional usage of condoms(OR=4.341,P=0.000),male-male sexual behavior(OR=8.014,P=0.000),drunken sexual behavior(OR=2.504,P=0.004),tattoos(OR=3.309,P=0.000)and commercial sexual behavior(OR=3.197,P=0.000)were independent risk factors of syphilis infection.Conclusions The number of sexual partners>2,no/occasional usage of condoms,male-male sexual behavior,drunken sexual behavior,tattoos and commercial sexual behavior are independent risk factors of syphilis infection in Xixian New District,Xi′an City.Clinically,preventive measures can be formulated from these aspects.
