基于决策树模型的复方沙棘籽油栓治疗老年性阴道炎的药物经济学评价

摘　　要：目的 评价复方沙棘籽油栓(CSSOS)治疗老年性阴道炎(SV)的经济性,以期为卫生医疗用药资源的合理配置提供参考依据。方法 采用TreeagePro软件对妇炎舒胶囊单用vs妇炎舒胶囊+CSSOS干预SV进行成本-效果分析,成本参数来源于米内网数据库和相关文献,同时根据一项已发表的随机对照研究获得的效果参数(临床有效率)评价两种治疗方案短期内的经济性,模型模拟的干预时间为12d。通过单因素敏感性分析(旋风图)、概率敏感性分析(增量成本-效果散点图、成本-效果可接受曲线),对研究结果的稳健性进行验证。结果 基础分析结果显示,两组的增量成本-效果比(ICER)值为382.44元,即相比于单用妇炎舒胶囊,妇炎舒胶囊联合CSSOS每多治疗1例有效的SV患者需多花费382.44元,当患者的个人意愿支付(WTP)价格高于382.44元时,CSSOS联合妇炎舒胶囊组具有经济性的概率优于单用妇炎舒胶囊组。敏感性分析结果验证了基础分析结果的稳健性。结论 假定人均可支配收入(2020年)作为患者的个人WTP价格,SV患者选用CSSOS联合妇炎舒胶囊比单用妇炎舒胶囊具有经济性的概率更高。Objective To assess the economy of Compound Seabuckthorm Seed Oil Suppository(CSSOs)in the treatment of senile vaginitis(SV),so as to provide reference for rational allocation of medical medication resources.Methods In this study,Treeage Pro software was used to analyze the cost-effectiveness of CSSOS+Fuyanshu Capsule VS Fuyanshu Capsule in the intervention of Sv.The cost parameters were obtained from the Menet database and relevant literature.The short-term economics of the two regimens were evaluated based on an efficacy parameter(clinical effective rate)derived from a published randomized controlled study.The time span of simulation was 12 days.The robustness of this study was verified by single factor sensitivity analysis(whirlwind plot),probabilistic sensitivity analysis.Results The results of basic analysis showed that the ICER of the CSSOS group and the control group was 382.44 yuan.That is,compared with the Fuyanshu Capsule alone,the CSSOS combined with Fuyanshu Capsule would cost 382.44 yuan more for each additional patient with effective SV.At this time,if the patient's willing to pay more than 382.44 yuan,CSSOS and Fuyanshu Capsule is more economical.Otherwise,Fuyanshu Capsule alone is more economical.Sensitivity analysis shows that the basic analysis results of this study are robust.Conclusion Based on the assumption that per capita disposable income in 2020 is the threshold of patients'willingness to pay,it is more economical to use CSSOS combined with Fuyanshu Capsule than Fuyanshu Capsule alone for patients with SV.
