摘 要:目的 分析干预前后Ⅰ类切口围术期抗菌药物的预防使用情况,为临床Ⅰ类切口围术期合理预防使用抗菌药物提供参考。方法 选取湖南中医药高等专科学校附属第一医院/湖南省直中医医院2020年3—6月(干预前,1 430例)和2021年3—6月(干预后,1 918例)Ⅰ类切口围术期预防使用抗菌药物的患者,比较干预前后Ⅰ类切口围术期抗菌药物预防使用率、病历合格率、原则上不预防使用抗菌药物比例及切口感染率。结果 干预前后医院Ⅰ类切口围术期抗菌药物预防使用率分别为27.13%、26.69%(P>0.05);病历合格率分别为82.87%、89.42%(P<0.05),原则上不预防使用抗菌药物比例分别为19.64%、3.91%(P<0.05);切口感染率分别为1.75%、0.99%(P>0.05)。结论 通过干预医院Ⅰ类切口围术期抗菌药物预防使用率下降,病历合格率提高,原则上不预防使用抗菌药物比例及切口感染率下降,但仍存在预防用药不合理情况。Objective To analysis the prophylactic use of antibacterials in perioperative period of dass Ⅰ incision before and after intervention, so as to provide reference for rational prophylactic use of antibacterials in perioperative period of class Ⅰ incision. Methods The perioperative prophylactic use of antibiotics in class Ⅰ incisions in the First Affiliated Hospital of Hunan College of Traditional Chinese Medicine/Hunan Provincial Hospital of Traditional Chinese Medicine from March to June in 2020(1 430 cases before intervention) and from March to June in 2021(1 918 cases after intervention) were selected. The perioperative antibiotic prophylactic use rate, the qualified rate of medical record, proportion of non prophylactic use of antibiotics in principle and incision infection rate of class Ⅰ incision before and after the intervention were compared. Results Before and after the intervention, the perioperative antibiotic prophylaxis rates of class Ⅰ incisions in the hospital were 27.13% and 26.69% respectively(P>0.05), the qualified rates of medical records were 82.87% and 89.42% respectively(P<0.05), in principle, the proportion of non preventive use of antibiotics was 19.64% and 3.91% respectively(P<0.05);The infection rates of incisions were 1.75% and 0.99% respectively(P>0.05). Conclusion Through the intervention of the hospital, the perioperative use rate of antibacterial drugs to prevent class Ⅰ incisions decreased, and the qualified rate of medical records increased. In principle, the proportion of non preventive use of antibacterial drugs and the rate of incision infection decreased, but there was still unreasonable use of preventive drugs.
