硝呋太尔制霉素联合乳杆菌活菌治疗孕晚期B族链球菌阳性并阴道微生态异常患者的临床疗效

摘　　要：目的 观察硝呋太尔制霉素联合乳杆菌活菌治疗孕晚期B族链球菌(GBS)阳性并阴道微生态异常患者的临床疗效。方法 将2018年12月—2020年12月于南京医科大学康达学院附属涟水县人民医院接受治疗的98例GBS并阴道微生态异常的孕晚期患者按照随机数字表法分为对照组和观察组,各49例。对照组予以硝呋太尔制霉素治疗,观察组则在对照组基础上加用乳杆菌活菌。2组均持续治疗7 d。比较2组临床疗效,治疗前及治疗7 d后阴道pH值、乳杆菌正常率、细菌多样性正常率及不良妊娠结局、药物不良反应发生率。结果 治疗7 d后,观察组治疗总有效率为87.75%,高于对照组的65.31%(χ^(2)=6.874,P=0.009)。治疗7 d后,2组阴道pH值低于治疗前,乳杆菌正常率、菌群多样性正常率均高于治疗前,且观察组变化幅度大于对照组(P<0.01)。观察组不良妊娠结局总发生率为4.08%,低于对照组的18.37%(χ^(2)=5.018,P=0.025)。2组治疗期间头晕恶心、外阴灼热、阴道干涩发生率比较,差异无统计学意义(P>0.05)。结论 硝呋太尔制霉素联合乳杆菌活菌治疗孕晚期GBS阳性并阴道微生态异常患者的效果较好,可改善阴道内环境,保障阴道微生态平衡,进而改善母婴结局,且安全性较高。Objective To explore the clinical effect of nifuratel nystatin combined with live lactobacillus in the treatment of late stage of pregnancy patients with positive group B streptococcus(GBS)and vaginal microecological abnormality.Methods A total of 98 cases of late stage of pregnancy patients with positive GBS and vaginal microecological abnormality were selected from December 2018 to December 2020 in Nanjing Medical University Kangda College Affiliated Lianshui County People′s Hospital,and they were divided into the control group and the observation group by the random number table method,49 cases in each group.The control group was given nifuratel nystatin,while the observation group was additionally given live lactobacillus on the basis of the control group,both groups were treated for 7 days.Clinical effect,vaginal pH value,normal rate of lactobacillus,normal rate of flora diversity before and after 7 days of treatment,adverse pregnancy outcomes and incidence of adverse reactions were compared between the two groups.Results The total effective rate of the observation group after 7 days of treatment was 87.75%,which was higher than 65.31%of the control group(χ^(2)=6.874,P=0.009).After 7 days of treatment,vaginal pH value of the two groups was lower than that before treatment,normal rate of lactobacillus,normal rate of flora diversity were higher than those before treatment,and those of the observation group were better than control group(P<0.01).The incidence rate of adverse pregnancy outcomes of the observation group was 4.08%,which was lower than 18.37%of the control group(χ^(2)=5.018,P=0.025).There was no significant difference of the incidence of adverse reactions between the two groups(P>0.05).Conclusion Nifuratel nystatin combined with live lactobacillus have a better effect in the treatment of late stage of pregnancy patients with positive GBS and vaginal microecological abnormality,it can improve the vaginal environment,ensure the vaginal microecological balance,and then improve the pregnancy outcom
