清代名医何梦瑶论痰及治疗胸痹心痛类病证之特色探析

摘　　要：清代名医何梦瑶善于论治胸痹心痛类病证,其学术特色在岭南医学界有广泛的影响,具有重要的研究价值。通过对何梦瑶代表著作《医碥》及相关文献的研究,发现何梦瑶论治胸痹心痛类病证时尤其注重从痰论治,并对中医痰证多有阐发。其论痰特色主要可概括为:擅长痰证的寒热辨证,主张调胃气以治痰,发挥痰生窠囊说,强调痰火郁三者相关。在论治胸痹心痛类病证时,何梦瑶对胸痹、痞满、心痛、胸痛等病证进行鉴别比较、条分缕析,从痰证角度予以论治,颇具特点,其学术特色可供现代医家论治冠心病特别是痰瘀互结型冠心病参考。He Mengyao,a famous doctor in the Qing Dynasty,was good at treating chest arthralgia and heartache.His academic characteristics had a wide influence in the Lingnan medical community and had important research value.Through a study of Yibian and related literatures,we found that special emphasis was placed on phlegm theory treatment when He Mengyao treated chest discomfort and heart pain,and he also focused on the elucidation of the theory of phlegm.He Mengyao’s theory of phlegm characteristics can be mainly summarized as follows:emphasis on cold-heat syndrome differentiation of phlegm syndrome,proposition of modulating stomach Qi to treat phlegm,exerting the theory of Ke-nang,emphasizing the correlation of phlegm,fire and depression.In discussing the treatment of chest discomfort and heart pain,He Mengyao carried out differential comparisons between the diseases such as chest discomfort,distention and fullness,heart pain and chest pain,and was quite characteristic in treating them from phlegm.Its academic characteristics can be used by modern physicians for treating coronary heart disease,especially the syndrome of phlegm and blood stasis binding pattern.
