摘　　要：出院计划作为一种医护患共同参与的新型延续性护理模式,对医疗资源优化及医疗质量的提升具有重要意义,其在成人慢病管理中的应用较广泛。目前,国内对早产儿出院计划的研究仍处于初步探索阶段。本文阐述出院计划的内涵以及近年来国内外早产儿出院计划的研究现状,分析目前我国早产儿出院计划实施的影响因素,旨在促进早产儿出院计划的推进和发展。As a new type of transitional care mode involving doctors,nurses and patients,discharge plan is of great significance to the optimization of medical resources and the improvement of medical quality.It is widely used in adult chronic disease management.At present,domestic research on discharge plan of premature infants is still in the preliminary exploration stage.This paper expounds the connotation of discharge plan and the research status of discharge plan for premature infants at home and abroad in recent years,and analyzes the factors affecting the implementation of discharge plan for premature infants in China,aiming at promoting the promotion and development of discharge plan for premature infants.
