摘  要:目的评价心脏磁共振(CMR)在系统性硬化病(SSc)中的临床应用价值。方法回顾性分析进行CMR检查的SSc患者62例,评价其心脏形态学、功能学及心肌延迟强化(LGE)特征改变。结果39例SSc患者出现1种或以上CMR异常表现,其中29例心脏形态学改变,4例心脏功能学改变,21例心肌LGE。21例心肌LGE均位于肌壁间,呈斑片状或线状,分布于左室中段或基底段。CMR异常和心肌LGE与SSc患者的临床分型无统计学相关性,而与患者的年龄和病程相关(P<0.05)。心律失常组的心肌LGE阳性率高于正常组(73%和31%,P<0.05)。结论CMR可检出SSc患者心脏形态学、心脏功能学的改变以及心肌LGE,对于SSc患者心脏受累的诊断具有较高的临床应用价值。Objective To evaluate the clinical application value of cardiac magnetic resonance(CMR)in systemic sclerosis(SSc).Methods Sixty-two patients with SSc undergoing CMR examination were analyzed retrospectively.The changes of cardiac morphological,function and myocardial late gadolinium enhancement(LGE)were evaluated.Results Thirty-nine patients with SSc had one or more CMR abnormalities,including 29 patients of cardiac morphological changes,4 patients of cardiac function changes and 21 patients of myocardial LGE.All 21 patients of myocardial LGE were intramural,patchy or linear,and distributed in the middle or basal segment of the left ventricle.CMR abnormalities and myocardial LGE had no statistical correlation with the clinical classification of SSc patients,but were associated with patient age and course of disease(P<0.05).Patients with arrhythmia had a greater probability of myocardial LGE in comparison to those with normal heart rhythm(73%versus 31%,P<0.05).Conclusion CMR can identify cardiac morphological and function changes and myocardial LGE,and it is a valuable tool for the diagnosis of cardiac involvement in patients with SSc.
