摘　　要：目的评价中心静脉CT静脉造影(CTV)对肾透析患者静脉通路狭窄的诊断效能。方法选取50例维持性血液透析(MHD)患者,对所有患者CTV检查后再进行数字减影血管造影(DSA)检查,统计CTV与DSA检查结果的一致性,评价宽体探测器中心静脉扫描模式下的诊断效能。结果CTV共检出70条狭窄或闭塞的血管,与DSA检出数一致。CTV检查结果与DSA的检查结果对于中心静脉狭窄评估比较差异无统计学意义(管腔轻度、中度、重度狭窄及闭塞准确率对比统计,P>0.05),通过对患者的CTV检查结果效能进行分析,CTV检查的灵敏度100%(70/70),特异度91.43%(64/70),准确度100%(70/70)。结论宽体探测器CT在中心静脉CTV检查中扫描速率快,与DSA比较对于静脉血管捕捉更为全面,对于血液透析通路狭窄的患者在治疗前给临床可以提供准确的影像资料进行评估。Objective To evaluate the diagnostic efficacy of central venous computed tomography venography(CTV)for venous access stenosis in renal dialysis patients.Methods A total of 50 patients with maintenance hemodialysis(MHD)were selected for digital subtraction angiography(DSA)examination after the CTV examination,and the consistency of CTV and DSA examination results was counted.The diagnostic performance of wide-body detectors in central vein scanning mode was evaluated.Results A total of 70 narrow or occluded blood vessels were detected by CTV,which was the same as the number detected by DSA.There was no statistically significant difference between the results of CTV and DSA for the evaluation of central venous stenosis(compare statistics of the accuracy of mild,moderate,and severe stenosis and occlusion of the lumen,P>0.05).The results of the CTV examination were evaluated based on the effectiveness of the results.Analysis showed that the sensitivity of the CTV examination was 100%(70/70),the specificity was 91.43%(64/70),and the accuracy was 100%(70/70).Conclusion A wide-body detector CT is used in the central venous CTV examination.The scanning rate is fast,and compared with DSA,it is more comprehensive in capturing venous blood vessels.For patients with hemodialysis access stenosis,it can provide accurate imaging data for clinical evaluation before treatment.
