摘　　要：目的探讨女性盆腔器官脱垂(POP)动静态MR耻骨骶尾关节线(PCLjnt)、H线及其代表的分级系统在诊断POP中的应用价值及对比分析。方法回顾性分析经临床诊断为POP,且行动静态MR检查的56例患者。分别采用PCLjnt线及H线评估患者的前、中、后盆腔的脱垂程度,比较2种不同分度方法与临床盆腔器官脱垂量化分期(POP-Q)分度的结果;并采用Spearman秩相关分析及Kappa一致性检验,比较2种分度方法在前、中盆腔脱垂分度的差异。结果以POP-Q为参照标准,PCLjnt线在前、中、后POP的阳性检出率分别为83.33%(45/54)、96.36%(53/55)、100%(36/36),H线在前、中、后POP的阳性检出率分别为75.93%(41/54)、61.82%(34/55)、100%(36/36)。PCLjnt线与H线在前、中POP诊断都具有极强相关性(ρ分别为0.859、0.817,P<0.01)。PCLjnt线与H线在前POP诊断具有中等一致性,在中POP诊断具有较低一致性(k分别为0.540、0.346,P<0.01)。结论PCLjnt线与H线在前、中POP诊断具有极强相关性,分别具有中等一致性及较低一致性,PCLjnt线比H线具有更高阳性检出率;H线比PCLjnt线可独特评估盆底松弛。Objective To investigate the application value and comparative analysis of pubic sacrococcygeal joint line(PCLjnt),H line and the grading system they represented in the diagnosis of female pelvic organ prolapse(POP).Methods Fifty-six patients with POP diagnosed clinically and examined by dynamic and static MR were analyzed retrospectively.PCLjnt line and H line were used to evaluate the degree of anterior,middle and posterior pelvic prolapse,and the results of two different grading methods and clinical pelvic organ prolapse quantification(POP-Q)grading were compared.S pearman rank correlation analysis and Kappa consistency test were used to compare the difference between the two methods in the degree of anterior and middle pelvic prolapse.Results With POP-Q as the reference standard,the positive detection rates of PCLjnt line in anterior,middle and posterior POP were 83.33%(45/54),96.36%(53/55)and 100%(36/36)respectively.The positive detection rates of H line in anterior,middle and posterior POP were 75.93%(41/54),61.82%(34/55)and 100%(36/36)respectively.There was a strong correlation between PCLjnt line and H line in the diagnosis of anterior and middle POP(ρwere 0.859 and 0.817,P<0.01).PCLjnt line and H line had moderate consistency in the diagnosis of anterior POP,and low consistency in the diagnosis of middle POP(k were 0.540 and 0.346,P<0.01).Conclusion PCLjnt line and H line have a strong correlation in the diagnosis of anterior and middle POP,with medium consistency and low consistency respectively.PCLjnt line has a higher positive detection rate than H line;H line can uniquely evaluate pelvic floor relaxation than PCLjnt line.
