摘 要:目的探讨MRI水-脂分离技术定量分析强直性脊柱炎(AS)患者骶髂关节(SIJ)骨髓脂肪分数(FF)及R2^(*)值变化的可行性,并分析FF与R2^(*)的关系。方法纳入75例AS患者,其中早期急性组24例,晚期急性组25例和稳定组26例。所有患者行IDEAL-IQ及常规SIJ MRI检查,测量AS患者骨髓水肿(BME)、脂肪沉积(FM)病灶的FF、R2^(*)值,其中39例AS患者含有BME(早期急性组15例,晚期急性组16例,稳定组8例)和39例AS患者含有FM(早期急性组9例,晚期急性组17例,稳定组13例)。对比分析不同时期AS患者BME、FM病灶FF、R2^(*)值的差异,并对FF与R2^(*)值进行相关性分析。结果患者组间BME内FF值差异有统计学意义(P=0.001),而BME内R2^(*)值无统计学意义;患者组间FM内FF、R2^(*)值均无统计学意义;FM内FF与R2^(*)值呈负相关(P=0.000,r=-0.488),而BME内FF与R2^(*)值间无相关性。结论FF、R2^(*)值可以量化SIJ不同时期BME、FM病灶脂肪含量、分析骨小梁微结构变化,为AS骨代谢异常变化提供无创、精准的影像指标。Objective To investigate the feasibility and correlation between sacroiliac j oint(SIJ)bone marrow fat fraction(FF)and R2^(*)value in patients with ankylosing spondylitis(AS)via chemical shift-encoded MRI.Methods Seventy-five AS patients,24 in the early acute group,25 in the late acute group,and 26 in the inactive group were enrolled.All patients underwent IDEAL-IQ and routine clinical SIJ MRI.FF and R2^(*)value were measured in bone marrow edema(BME)and fat metaplasia(FM).Out of those,39 with BME lesions(early acute group 15,late acute group 16,inactive group 8)and 39 with FM lesions(early acute group 9,late acute group 17,inactive group 13)were included.Differences in FF,R2^(*)value for BME,FM lesions from different stages were compared and the relationship between FF and R2^(*)value was analyzed.Results FF value for BME was significantly different among patient groups(P=0.001),while R2^(*)value for BME showed no difference.No difference was observed in neither the FF nor R2^(*)value in FM lesions among the patient groups.FF was negatively correlated with R2^(*)value in FM(P=0.000,r=-0.488),but there was no correlation between FF and R2^(*)value in BME.Conclusion FF and R2^(*)value can quantitatively analyze the chan-ges in fat content and bony trabecular micro-structure for BME and FM lesions in different stage SIJ,providing non-invasive and accurate imaging markers for AS bone metabolic abnormalities.
