摘 要:目的探讨儿童克罗恩病(CD)在CT小肠成像(CTE)的影像特征及临床应用价值。方法回顾性分析29例经消化内镜和病理证实的CD影像资料,对其CTE特征进行分析。结果以临床表现、肠镜和病理组织学结果等检查综合判断为金标准,29例在CTE上均可见肠壁增厚及肠段壁强化,其中14例(48.3%)为分层强化;16例(55.2%)出现21处肠腔狭窄;18例(62.1%)可见溃疡;16例(55.2%)出现“木梳征”,该征象也是CD的特征表现之一;11例(37.9%)出现“脂肪爬行征”。结论儿童CD在CTE上可显示发病部位,肠壁情况(溃疡、肠腔狭窄、肠壁增厚)、“木梳征”及“脂肪爬行征”等特征,CTE表现有利于诊断CD及观察肠外表现。Objective To explore the imaging characteristics and clinical application value of CT enterography(CTE)in children’s Crohn’s disease(CD).Methods The 29 cases of patients with CD confirmed by endoscopy and pathology were analyzed retrospectively,and its CTE characteristics were analyzed.Results Clinical manifestation,colonoscopy,and histopathology were taken as the gold standards.All 29 cases of CTE showed intestinal wall thickening and segmental wall enhancement,of which 14 cases(48.3%)showed stratified enhancement.There were 21 intestinal strictures in 16 cases(55.2%);ulceration were seen in 18 cases(62.1%);“wood comb sign”was seen in 16 cases(55.2%),which was also one of the characteristic manifestations of CD;the“fat crawling sign”was seen in 11 cases(37.9%).Conclusion The location,intestinal wall conditions(ulceration,intestinal strictures,intestinal wall thickening),“wood comb sign”and“fat crawling sign”et al are show in the CTE of children’s CD.The CTE manifestations is helpful for the diagnosis of CD and the observation of extra-intestinal manifestations.
