摘　　要：目的探讨坐骨耻骨联合症(IPS)的影像学表现。方法收集坐骨耻骨联合部影像学呈阳性改变患儿45例。临床以下肢疼痛、跛行、发热、淋巴结炎、遗尿、性早熟、呼吸道感染、头及胸部外伤等就诊。全部病例均行髋关节MR平扫、骨盆X线检查,2例行髋关节MR增强扫描,5例行盆腔CT扫描,分析其影像学特点。结果45例患儿中6例双侧发病,39例单侧发病,其中右侧23例,左侧16例。MR显示坐骨耻骨联合部骨髓水肿,T_(2)WI压脂序列呈高信号,联合部中央“纤维带”T_(2)WI压脂呈低信号,垂直于耻骨下支轴位方向,部分邻近关节伴有软组织水肿。X线及CT表现为坐骨耻骨联合部局部膨隆,内部密度不均匀,中央可见圆形透亮区,周围有硬化缘。结论IPS是坐骨耻骨融合前骨的正常变异,通常在青春期之前发生,骨表现有轻度膨隆、增大,“纤维带”是IPS在MR上的特征性表现。Objective To investigate the imaging findings of ischiopubic synchondrosis(IPS).Methods Forty-five cases with positive imaging performance of ischio pubic symphysis were collected.Clinical complaints included lower limb pain,claudication,fever,lymphadenitis,enuresis,precocious puberty,respiratory tract infection,head and chest trauma.All cases underwent plain MR scan of hip joint and pelvic X-ray,2 cases underwent enhanced MR scan of hip j oint and 5 cases underwent pelvic CT scan.The imaging characteristics were analyzed.Results Among the 45 cases,6 cases were bilateral and 39 cases were unilateral,including 23 cases on the right and 16 cases on the left.MR showed high signal in T_(2)WI fat saturation sequence in ischial pubic symphysis that indicates the bone marrow edema,and the central“fibrous band”of symphysis was low signal in T_(2)WI fat saturation sequence,perpendicular to the axial direction of the inferior pubic branch,and some adj acent j oints were accompanied by soft tissue edema.X-ray and CT showed local bulge at the ischio pubic symphysis,heterogenous internal density,circular transparent area in the center and sclerotic edge around.Conclusion IPS is a normal variation before sciatic pubic fusion,which usually occurs before puberty.The bone shows mild swelling and enlargement.“Fibrous band”is the characteristic findings of IPS on MR.
