摘 要:目的探讨癫痫(EP)患儿血清miR-142、PTEN诱导假定激酶1(PINK1)的表达水平及临床意义。方法选取2019年3月至2021年4月期间我院神经内科收治的79例EP患儿为研究组,根据病情严重程度分为轻度EP组(n=40)、中度EP组(n=19)和重度EP组(n=20);另选取同期于我院保健科体检的83例健康儿童为对照组。采用实时荧光定量PCR(qRTPCR)法测定所有受试者血清miR-142、PINK1mRNA的表达水平;采用受试者工作特征(ROC)曲线分析血清miR-142、PINK1 mRNA对EP患儿的诊断价值;多因素Logistic回归分析血清miR-142、PINK1mRNA对EP的影响效应。结果与对照组比较,研究组患儿血清miR-142水平明显较高,血清PINK1mRNA水平明显较低(t值分别为12.880和9.460,P<0.05)。不同EP类型患儿血清miR-142、PINK1mRNA水平比较,差异无统计学意义(P>0.05)。与轻度EP组比较,中度EP组、重度EP组患儿血清miR-142水平明显升高,血清PINK1mRNA水平明显降低(P<0.05);与中度EP组比较,重度EP组患儿血清miR-142水平明显升高,血清PINK1mRNA水平明显降低(P<0.05)。ROC曲线分析显示,miR-142、PINK1mRNA诊断EP患儿的曲线下面积(AUC)分别为0.845(95%CI:0.786~0.904)、0.791(95%CI:0.720~0.862),最佳诊断临界值分别为1.569、0.813,敏感度分别为78.2%、72.9%,特异性分别为84.5%、83.2%;二者联合检测诊断EP患儿的AUC为0.915(95%CI:0.870~0.960),敏感度为91.9%,特异性为82.7%。多因素Logistic分析显示,miR-142水平是影响EP发生的危险因素(OR=1.980,95%CI:1.119~3.502),PINK1mRNA水平是影响EP发生的保护因素(OR=0.892,95%CI:0.809~0.984)。结论EP患儿的血清miR-142呈高表达,PINK1呈低表达,二者均可作为EP诊断的辅助指标。Objective To explore the expression levels and clinical significance of serum miR-142 and PTEN induced putative kinase 1(PINK1) in children with epilepsy(EP).Methods 79 children with EP admitted to the Department of Neurology in our hospital from March 2019 to April 2021 were selected as the study group.According to the severity of the disease, they were divided into the mild EP group(n=40),the moderate EP group(n=19),and the severe EP group(n=20).In addition, 83 healthy children who were examined in the health care department of our hospital during the same period were selected as the control group.Quantitative real-time PCR(qRT-PCR) method was used to determine the expression levels of serum miR-142 and PINK1 mRNA in all subjects.Receiver operating characteristic(ROC) curve was used to analyze the diagnostic value of serum miR-142 and PINK1 mRNA for EP patients.Multivariate Logistic regression was used to analyze the effect of serum miR-142 and PINK1 mRNA on EP.Results Compared with the control group, the level of serum miR-142 in the study group was significantly increased and the level of serum PINK1 mRNA was significantly decreased(t=12.880 and 9.460,respectively, P<0.05).There were no significant difference in the levels of serum miR-142 and PINK1 mRNA among children with different EP types(P>0.05).Compared with mild EP group, the level of serum miR-142 in moderate EP group and severe EP group was significantly increased, and the level of serum PINK1 mRNA was significantly decreased(P<0.05).Compared with moderate EP group, the level of serum miR-142 in severe EP group was significantly increased, and the level of serum PINK1 mRNA was significantly decreased(P<0.05).ROC curve analysis showed that the area under the curve(AUC) of miR-142 and PINK1 mRNA in diagnosing EP children was 0.845(95%CI:0.786-0.904) and 0.791(95% CI:0.720-0.862),respectively, the best diagnostic cut-off value was 1.569 and 0.813,respectively, and the sensitivity was 78.2% and 72.9%,respectively, the specificity was 84.5% and 83.2%,resp
