摘 要:目的探讨美罗培南脑室内注射对颅脑损伤术后颅内感染患者脑脊髓液生化指标水平的影响。方法采用随机数字表法将丰城市人民医院2021年1月-2022年5月收治的颅脑损伤术后颅内感染患者70例分为两组,每组35例。对照组采用美罗培南静脉滴注治疗,观察组患者采用脑室内注射美罗培南治疗。观察两组临床疗效、脑脊液生化指标水平[总蛋白水平(PC)、白细胞计数(WBC)、葡萄糖水平(GS)]、炎症因子水平[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)和血清C-反应蛋白(CRP)水平]和不良反应。结果与对照组相比,观察组治疗后总有效率较高(P<0.05);与对照组相比,观察组治疗后WBC、PC水平较低,GS水平较高(P<0.05);与对照组相比,观察组治疗后IL-6、TNF-α和CRP水平均较低(P<0.05);两组不良反应比较,无统计学差异(P>0.05)。结论美罗培南脑室内注射治疗颅脑损伤术后颅内感染有助于调节患者脑脊液生化指标水平,减轻炎症反应,疗效显著,具有较高的安全性,值得临床推广应用。Object ive To investigate the effect of intravascular meropenem injection on the biochemical indexes of cerebrospinal fluid(CSF)in patients with intracranial infection after craniocerebral injury.Methods 70 patients with intracranial infection after craniocerebral injury in our hospital from January 2021 to May 2022 were randomly divided into two groups,35 patients in each group.The control group was treated by intravenous drip of meropenem,and the patients in the observation group were treated by intraventricular injection of meropenem.Observe the clinical efficacy,CSF biochemical index level[total protein level(PC),white blood cell count(WBC),glucose level(GS)],and inflammatory factor level[tumor necrosis factor-α(TNF-α),Interleukin-6(IL-6)and serum C-reactive protein(CRP)levels]and adverse reactions.Results Compared with the control group,the total effective rate of the observation group was higher(P<0.05);compared with the control group,the levels of WBC and PC in the observation group were lower and the levels of GS were higher after treatment(P<0.05);compared with the control group,IL-6 and TNF-αin the observation group after treatment and CRP levels were lower(P<0.05);there was no significant difference between the two groups(P>0.05).Conclusion The intraventricular injection of meropenem in the treatment of intracranial infection after craniocerebral injury is helpful to regulate the level of biochemical indicators of cerebrospinal fluid,reduce inflammatory reaction,has significant efficacy and high safety,and is worthy of clinical application.
