PDCA循环法联合集束化干预在恶性血液病患者化疗后口腔溃疡中的应用

摘　　要：目的应用PDCA循环模式+集束化护理干预后,对恶性血液病患者化疗后口腔溃疡的应用效果进行评价。方法选择贵港市人民医院2020年5月-2022年5月期间收治的恶性血液病所致口腔溃疡患者(n=58例)为研究对象,依据随机数字表法将入选患者分为观察组及对照组。对照组(n=29例)行常规口腔护理干预,观察组在常规口腔护理基础上行PDCA循环模式+集束化护理干预。观察干预后患者的口腔溃疡改善分级、护理满意度,在应用前后,对患者疼痛程度评分(VAS)、口腔功能评分(KPS)进行评估。结果观察组患者的住院时间更短,口腔溃疡改善分级、护理满意度更优,与对照组相比,组间数据有统计学意义(P<0.05)。干预前,两组VAS、KPS评分组间比较,无统计学意义(P>0.05);干预后,两组患者的VAS评分较干预前呈现明显降低趋势,KPS评分较干预前呈现增高趋势,且观察组患者上述评分改善幅度明显优于对照组,两组数据比较,具有统计学意义(P<0.05)。结论恶性血液病化疗后所致口腔溃疡患者经PDCA循环护理+集束化护理干预后,患者的口腔溃疡改善分级更优,可减轻疼痛程度,提高口腔功能及护理满意度,两种护理方法协同应用效果理想,可缩短患者的住院时间,干预安全有效。Objective The application effect of the PDCA circulation mode+cluster nursing intervention was evaluated in oral ulcers after chemotherapy in patients with hematological malignancies.Methods During May 2020 and May 2022,the patients(n=58 cases)of oral ulcer caused by hematological malignancy admitted to our hospital were selected as the study subjects,and the selected patients were divided into observation group and control group according to the random number table.The control group(n=29 cases)performed the routine oral care intervention,and the observation group performed the PDCA cycle pattern+cluster care intervention.Observe the grade of oral ulcer improvement and nursing satisfaction after the intervention,and then,the patient's pain degree score(VAS)and oral function score(KPS)were evaluated before and after the application.Results Patients in the observation group had shorter hospital stay,better grade of oral ulcer improvement and beter satisfaction of care,with statistically significant data compared to the control group(P<0.05).Before the intervention,VAS and KPS scores were not significant(P>0.05),the VAS scores were significantly lower,KPS scores were higher than before the intervention,and the observation group was significantly better than the control group and statistically significant(P<0.05).Conclusion After PDCA circulation chemotherapy+cluster care intervention,patients'oral ulcer improvement grade is better,which can reduce the degree of pain,improve oral function and nursing satisfaction.The coordination effect of the two nursing methods is ideal,which can shorten the hospital stay of patients,and the intervention is safe and effective.
