摘　　要：目的分析垂直切口内侧蒂式乳房缩小整形术的疗效及并发症发生情况。方法本研究选取河南整形美容医院2020年6月至2022年7月收治的94例乳房肥大症患者,所有患者均接受垂直切口内侧蒂式乳房缩小整形术治疗,观察其围术期指标,并评估其手术前后乳房相关指标变化、乳房下垂严重程度及生活质量改善状况。结果94例患者单侧乳房切除的组织平均重量达到(1185.43±16.79)g,单侧脂肪抽吸量平均值为(86.52±3.06)mL;术后共有4例患者出现皮下血肿的状况,并发症发生率为4.26%,未发生严重并发症(包括感染、乳头乳晕感觉减退、坏死等);术后平均拆线时间为(13.41±1.08)d;有6例患者出现瘢痕增生情况,温哥华瘢痕评定量表平均得分为(3.18±0.24)分;问卷调查患者对乳房外形美观度的满意度平均值为(9.73±0.12)分。患者术后的左、右锁乳线,左、右胸乳线,乳晕直径及过乳头胸围均低于术前,差异有统计学意义(P<0.05)。患者术后乳房下垂状况明显优于术前,差异有统计学意义(P<0.05)。患者术后简易生活质量量表(SF-36)各维度得分高于术前,差异有统计学意义(P<0.05)。结论垂直切口内侧蒂式乳房缩小整形术用于临床乳房肥大症治疗中可获得确切疗效,其并发症发生情况较少。Objective To analyze the clinical effects and complications of the vertical incision medial pedicled breast reduction.Methods A total of 94 mammary hypertrophy patients of undergoing the vertical incision medial pedicled breast reduction from June 2020 to July 2022 in Henan Plastic and Cosmetic Hospital were selected.The perioperative indicators,breast indicators,mastoptosis severity and patient’s life quality before and after surgical operation were assessed.Results In 94 patients,the average resected unilateral breast tissue weight was(1185.43±16.79)g;the mean unilateral fat suction volume was(86.52±3.06)mL.After surgery,there were 4 cases of subcutaneous hematoma(4.26%);the serious complications like infections,papilla and mammary areola hypaesthesia,necrosis were not occurred.The mean suture removal time was(13.41±1.08)d.There were 6 cases discovered with hypertrophic scar.The mean Vancouver scar scale scores were(3.18±0.24);The mean satisfaction scores of breast appearance were(9.73±0.12).The left and right lock line,left and right breast line,mammary areola diameter,chest circumference were significantly lower than before,with significant differences(P<0.05);The mastoptosis severity after surgery was significantly better than before,with significant difference(P<0.05);The Mos 36-item Short Form Health Survey(SF-36)scores were significantly higher than before,with significant differences(P<0.05).Conclusion The vertical incision medial pedicled breast reduction can effectively increase the curative effects and reduce the surgical complications for mammary hypertrophy patients.
