高位结扎加剥脱术治疗大隐静脉曲张的临床疗效

摘　　要：目的研究高位结扎加剥脱术治疗大隐静脉曲张的临床疗效。方法回顾性分析洛阳伊滨区李村镇卫生院2017年6月至2022年6月收治的62例大隐静脉曲张患者治疗情况,根据治疗手术方式差异分为两组。其中对照组31例,实施高位结扎加传统抽剥脱术治疗;研究组31例,实施高位结扎加剥脱术治疗。观察两组患者治疗期间及治疗后情况、创伤定量指标、手术前后生活质量调查表(CIVIQ)、并发症发生情况、复发率。结果研究组手术时间长于对照组,下床活动时间、住院时间更短,术中出血量更低。术前两组患者各项创伤定量指标[丙二醛(MDA)、超氧化物歧化酶(SOD)、一氧化氮(NO)、白细胞介素(IL-6)]比较差异无统计学意义(P>0.05);术后72 h研究组MDA、NO、IL-6水平低于对照组,SOD水平高于对照组,差异有统计学意义(P<0.05)。两组术前各项CIVIQ评分比较,差异无统计学意义(P>0.05);术后7 d,两组CIVIQ评分均出现明显提高,且研究组高于对照组,差异有统计学意义(P<0.05)。研究组术后并发症发生率(16.13%)低于对照组(48.39%),且研究组复发率(3.23%)低于对照组(16.13%),差异有统计学意义(P<0.05)。结论大隐静脉曲张患者,在实施高位结扎加剥脱术后,术中整体出血量较少,能够缩短患者下床及住院时间,降低术中创伤,提升术后生存质量,且患者并发症发生率及复发率均下降,术中及术后恢复情况较为理想,值得应用。Objective To study the clinical effectiveness of high ligation and stripping in treating the great saphenous varicose vein(GSVV).Methods A total of 62 GSVV patients treated from June 2017 to June 2022 in Licun Township Hospital,Yibin District,Luoyang were reviewed.According to different surgical modes,31 cases received the high ligation and traditional peeling were included in the control group,and 31 cases received the high ligation and stripping were included in the study group.The conditions of the two groups during and after treatment,quantitative trauma indicators,preoperative and postoperative quality of life questionnaire(CIVIQ),complications and recurrence rate were observed.Results The operation time of the study group was longer than that of the control group,and the time of getting out of bed,hospitalization were shorter,and intraoperative bleeding volume was even lower.There were no differences between the two groups in the quantitative indexes of trauma[malondialdehyde(MDA),superoxide dismutase(SOD),nitric oxide(NO),interleukin 6(IL-6)]before operation(P>0.05);The levels of MDA,NO and IL-6 in the study group were lower than those in the control group 72 hours after operation,while SOD was higher than that in the control group,with significant differences between the groups(P<0.05);There were no significant differences in CIVIQ scores between the two groups before operation(P>0.05).The CIVIQ scores of the two groups were significantly improved 7 days after operation,and those of the study group were higher than those of the control group,with significant differences between the groups(P<0.05);The total incidence of postoperative complications in the study group was 16.13%,lower than that in the control group(48.39%),and the recurrence rate in the study group was 3.23%,lower than that in the control group(16.13%),the differences between groups were significant(P<0.05).Conclusion For patients with greater saphenous varicose veins,after high ligation and stripping,the overall blood loss during the operat
