补益健脾祛湿方加减对老年股骨上段骨折术后延迟愈合患者骨代谢水平的影响

摘　　要：目的观察补益健脾祛湿方加减对老年股骨上段骨折术后延迟愈合患者骨代谢水平的影响。方法选取项城市中医院2021年6月至2022年6月收治的86例老年股骨上段骨折患者,根据随机数字表法将患者分为观察组(N=43例)与对照组(N=43例),两组入院后均给予自体骨移植术及骨髓血注射治疗,观察组再给予补益健脾祛湿方加减治疗。观察两组术后恢复情况及治疗前后X线骨折愈合程度评分、骨代谢水平[碱性磷酸酶(ALP)、骨钙素(OC)]、髋关节功能评分量表(Harris)评分变化,并统计两组临床疗效。结果观察组肿胀缓解时间、瘀斑消失时间及骨折愈合时间均短于对照组,差异有统计学意义(P<0.05)。治疗后,观察组X线骨折愈合程度评分、ALP、OC、Harris评分等均优于对照组,且观察组治疗有效率显著高于对照组,差异有统计学意义(P<0.05)。结论在自体骨移植术及骨髓血注射治疗基础上实施补益健脾祛湿方加减,不仅能促进老年股骨上段骨折患者术后骨折愈合,还能改善其骨代谢水平,对于其术后功能恢复正常也具有重要作用,可显著提高治疗效果。Objective To observe the effect of modified Buyi Jianpi Qushi Decoction on bone metabolism in elderly patients with delayed healing after upper femoral fracture surgery.Methods A total of 86 elderly patients with upper femoral fracture who were treated in the hospital from June 2021 to June 2022 were randomly divided into the observation group(N=43)and the control group(N=43).Both groups were treated with autologous bone graft and bone marrow blood injection.On this basis,patients in the treatment group were treated with modified Buyi Jianpi Qushi Decoction.Postoperative recovery and changes in X-ray fracture healing scores,bone metabolism level[alkaline phosphatase(ALP)and bone gla protein(OC)],pain degree and hip function(Harris)scores in the two groups were observed.Clinical effects in the two groups were analyzed.Results Swelling relief time,ecchymosis disappearance time and fracture healing time in the observation group were shorter than those in the control group(P<0.05).After treatment,X-ray fracture healing score,ALP,OC and Harris score in the observation group were better than those in the control group(P<0.05).The total response rate in the observation group was significantly higher than that in the control group(P<0.05).Conclusion Autologous bone graft and bone marrow blood injection combined with modified Buyi Jianpi Qushi Decoction can not only promote fracture healing but also improve bone metabolism in elderly patients with delayed healing after upper femoral fracture surgery.The combined treatment can also help to restore function after surgery and significantly improve therapeutic effects.
