无胆道镜辅助经脐单孔腹腔镜保胆取石术10例分析

摘　　要：目的:分析无胆道镜辅助行经脐单孔腹腔镜保胆取石术的临床应用价值。方法:回顾分析2020年1月至2021年6月无胆道镜辅助为10例胆囊结石患者行经脐单孔腹腔镜保胆取石术的临床资料,分析并总结经验与体会。结果:10例患者中男1例,女9例,平均(26.1±5.6)岁,BMI平均(24.5±2.3)kg/m^(2),单孔腹腔镜手术均获成功,无中转开腹。手术时间平均(61.0±13.2)min,术中出血少,术后未发现明显胆漏征象,患者均于术后3 d内出院。随访至术后12个月,美容效果佳,均未发现结石复发。结论:无胆道镜辅助经脐单孔腹腔镜保胆取石术是可行的,对有强烈保胆意愿、符合保胆手术适应证的年轻女性患者尤其适用。Objective:To analyze the clinical application value of transumbilical single-incision laparoscopic gallbladder-preserving lithotomy without the assistance of choledochoscope.Methods:From Jan.2020 to Jun.2021,the clinical data of 10 patients who underwent transumbilical single-incision laparoscopic gallbladder-preserving lithotomy without the assistance of choledochoscope were retrospectively analyzed,and the experience was summarized and analyzed.Results:Of the 10 patients,one was male and nine were female,with an averageage of(26.1±5.6)years and an average BMI of(24.5±2.3)kg/m^(2).All the single-port laparoscopic procedures were successful without conversion to laparotomy.The mean operation time was(61.0±13.2)min,intraoperative bleeding was little,and no apparent biliary leakage signs were found after the operation.All patients recovered quickly after surgery and were discharged within 3 d postoperatively.The follow-up of 12 months after the operation showed good cosmetic results,and no recurrence of stones was found in 10 patients.Conclusions:Transumbilical single-incision laparoscopic gallbladder-preserving lithotomy without the assistance of choledochoscope is feasible in clinical practice and is especially suitable for young female patients who have a strong desire to preserve gallbladder and meet the indications for biliary preservation surgery.
