腹腔镜盆底重建术对老年盆腔器官脱垂患者的临床疗效

摘　　要：目的 评价腹腔镜下盆底重建术对老年女性患者的临床疗效。方法 选择2017年11月-2021年11月在甘肃省妇幼保健院老年妇科因“盆腔器官脱垂Ⅱ~Ⅳ度”实施腹腔镜下盆底重建手术,年龄≥65岁的31例患者,记录围手术期指标,手术并发症采用Clavien-Dindo分级。术后以门诊检查、电话随访及问卷评价疗效。结果 术后3个月、6个月与术前相比,Aa、Ba、C、Ap、Bp、GH、PB、TVL均改善,有统计学差异(P <0.05)。随访25.90±17.08个月,复发率12.90%,未放置子宫托或再手术。整体印象改善评分为96.77%,围术期并发症发生率19.35%。结论 对于≥65岁的老年盆腔器官脱垂患者,腹腔镜下盆底重建手术是安全有效的。Objective To evaluate the clinical effect of laparoscopic reconstructive pelvic surgery in elderly patients with pelvic organ prolapse. Methods A retrospective cohort study was performed on 31 elderly patients who underwent laparoscopic reconstructive pelvic surgery between November 2017 and November2021. Only symptomatic uterine or vaginal vault prolapse patients with POP Q stage 2-4 were included.Baseline and intraoperative variables, perioperative complications of Clavien-Dindo Grade were determined.After the operation, the outpatient and telephone follow-up and questionnaires were used to evaluate the cases.Results After 3 and 6 months, points(Aa, Ba, C, Ap, Bp) and lines(GH, PB, TVL) were all improved(P <0.05). After 25.90±17.08 months’ follow-up, the recurrence rate was 12.90%. There was no reoperation and the patient global impression of improvement scale(PGI-I) was 96.77%. Perioperative complications occurred in 6 cases(19.35%). Conclusion Laparoscopic reconstructive pelvic surgery is safe and efficacious for woman patients ≥ 65 years with pelvic organ prolapse.
