从玄府理论探讨多发性骨髓瘤病机与治疗启发

摘　　要：本文通过梳理“玄府”内涵及源流、生理病理特点、目前中医对多发性骨髓瘤诊治的认识,提出现有认识无法含括多发性骨髓瘤前期病变及髓外病变等,结合玄府学说进行诊治可以弥补上述不足,提倡早期积极运用中医药防治疾病进展。玄府郁闭,尤其是肾、脾、肺之玄府病变是病机的关键,基于玄府贵开理论,强调治疗应重视宣通,提出直接开玄、通补兼施等治疗思想,为临床治疗提供参考。Through setting forth the connotation,historical development,physiological and pathological characteristics of Xuanfu(sweat pore)and by combining the present understanding of multiple myeloma(MM)in the field of traditional Chinese medicine(TCM),the article proposed that present TCM pathogenesis of MM can not fully explain the precancerous lesion of MM and its extramedullary disease.Xuanfu theory combined with present TCM pathogenesis can make up for the insufficiency.Treatment of TCM should be applied in early stage of MM to prevent the progression of the disease.The obstruction of xuanfu,especially the damage of Xuanfu of kidney,spleen and lung,is the key to pathogenesis of MM.For the function of xuanfu prefers being unobstructed,opening xuanfu during the treatment is stressed.The method of directly unblocking xuanfu and simultaneous use of dredging and tonifying therapy for MM presented in the article may be a new way for the treatment of MM.
