摘　　要：目的通过磁共振T2mapping成像探讨后纤维环与腰椎间盘突出的关系。方法对80例腰痛患者进行1.5T MRI检查,在正中矢状位T2图选取感兴趣区(region of interest,ROI),将腰椎间盘分为髓核(nucleus pulposus,NP)、前纤维环、纤维环的最后20%(20%of posterior annulus fibrosus,PAF-20)、纤维环的最后10%(10%ofposterior annulus fibrosus,PAF-10),比较其T2值的相关性。结果在400个椎间盘中,51个椎间盘突出,231个椎间盘膨出,118个正常椎间盘,据此分为三组。PAF-20的T2值在各组间差异均有统计学意义(P<0.01)。PAF-10的T2值方面,椎间盘突出组与正常椎间盘组有显著差异(P<0.01),但在椎间盘膨出组与椎间盘突出组之间无显著差异(P>0.05)。相关性分析显示,NP的T2值与PAF-20的T2值呈中度相关,与PAF-10的T2值呈弱相关。结论PAF-10及PAF-20的T2值降低,能够提示椎间盘突出。Objective To investigate the relationship between posterior annulus fibrosus and lumbar disc herniation via magnetic resonance imaging(MRI)T2 mapping.Methods Eighty patients with low back pain were examined by 1.5T MRI.Region of interest(ROI)analysis was performed on median,sagittal T2 map.Lumbar discs were dissected into nucleus pulposus(NP),anterior annulus fibrosus,20%of posterior annulus fibrosus(PAF-20),10%of posterior annulus fibrosus(PAF-10).The mean T2 values for PAF-20 and PAF-10 were compared and correlated with NP.Results In 400 discs,we found 51 herniated discs,231 bulging discs and 118 normal discs,and these patients were divided into three groups.T2 values of PAF-20 among three groups were statistically significantly different(P<0.01).There was significant differences in T2 value of PAF-10 between herniated disc group and normal disc group(P<0.01),but T2 value of PAF-10 had no significant difference between herniated disc group and bulging disc group(P>0.05).There was moderate association between the T2 value of PAF-20 and the T2 value of NP,but weak association between the T2 value of PAF-10 and the T2 value of NP.Conclusion Low T2 value of PAF-10 and PAF-20 suggest the presence of disc herniation.
