摘　　要：目的探讨应用零切迹椎间融合器(Zero-P)治疗跳跃型颈椎病的临床效果。方法回顾性分析2017年6月~2019年6月本院采用Zero-P椎间融合器治疗的跳跃型颈椎病患者21例。其中男12例,女9例;年龄45-72岁,平均(59.71±8.49)岁。采用JOA评分、疼痛VAS评分和颈椎NDI指数评价临床效果,采用C_(2-7)Cobb角评价颈椎曲度恢复情况,并记录术中术后并发症情况。结果21例患者均获随访12~36个月,神经功能均得到不同程度恢复,与术前相比,患者末次随访时的JOA评分、C_(2-7)Cobb角均显著提高(P<0.01),VAS评分、NDI指数均显著降低(P<0.01),差异均有统计学意义。1例患者术后出现吞咽困难。所有患者随访中无邻椎病、内置物沉降、螺钉松动、断裂和移位等并发症的发生,末次随访时均达骨性融合。结论采用颈前路椎间盘切除减压结合Zero-P椎间融合器置入治疗跳跃型颈椎病,可获得满意的临床效果。Objective To evaluate the surgical outcomes of Zero-P intervertebral fusion for skip cervical spondylosis.Methods From June 2017 to June 2019,the clinical data of 21 patients(12 males,9 females)with skip cervical spondylosis,aged 45-72 years old[average(59.71±8.49)years old],who underwent Zero-P intervertebral fusion were retrospectively analyzed.The JOA score,VAS scale and NDI index were used to evaluate pre-and post-operative clinical outcomes.The C_(2-7)Cobb angle was used to assess the recovery of cervical spine curvature.All intraoperative and postoperative complications were recorded.Results All 21 patients were followed up for 12-36 months,and their neurological function was restored to varying degrees.Compared with preoperation,the JOA score and C_(2-7)Cobb angle of the patients at the last follow-up were significantly increased(P<0.01),while the VAS score and NDI index were significantly decreased(P<0.01),with statistically significant differences.One patient had dysphagia after surgery.There were no complications such as adjacent spondylopathies,loosening of screws,breakage and displacement were observed during follow-up period.All patients achieved bony fusion at the last follow-up.Conclusion Zero-P intervertebral fusion can obtain satisfactory clinical results in the treatment of skip cervical spondylosis,and it is an alternative surgical treatment for skip cervical spondylosis.
